内镜下痔静脉套扎术和PPH治疗Ⅱ/Ⅲ度内痔的疗效比较

Comparison of endoscopic hemorrhoid vein ligation and PPH in thetreatment of Ⅱ/Ⅲ degree internal hemorrhoids

目的 比较内镜下痔静脉套扎术与吻合器痔上黏膜环切术(PPH)治疗Ⅱ/Ⅲ度内痔的疗效。方法 选择我院收治的Ⅱ/Ⅲ度内痔患者98例,将其随机分为观察组和对照组,每组49例。观察组采取内镜下痔静脉套扎术治疗,对照组采取PPH治疗;比较两组手术相关各项指标和术后并发症等,评价其临床疗效及安全性。结果 两组手术时间、住院时间、术中出血量、恢复时间、住院费用比较,差异有统计学意义(P<0.05)。两组疗效比较,差异无统计学意义(P>0.05)。两组肛周疼痛发生率比较,差异有统计学意义(P<0.05),两组术后出血、尿潴留、肛门狭窄、排便次数比较,差异无统计学意义(P>0.05)。结论 内镜下痔静脉套扎术治疗Ⅱ/Ⅲ度内痔具有创伤小、恢复快、花费低、并发症少等优势。

Objective To compare the efficacy of endoscopic hemorrhoidectomy and procedure for prolapse and hemorrhoids(PPH)in the treatment of gradeⅡ/Ⅲinternal hemorrhoids.Methods The clinical data of 98 patients with II/III degree internal hemorrhoids were collected and randomly divided into the observation group(n=49)and the control group(n=49).The observation group was treated with endoscopic hemorrhoid ligation,the control group was treated with PPH,and the related indexes and postoperative complications were compared between the two groups to evaluate the clinical efficacy and safety.Results There were significant differences in operation time,hospitalization time,blood loss,recovery time and hospitalization cost between the two groups(P>0.05).There was no significant difference in curative effect between the two groups(P>0.05),but there was significant difference in the incidence of perianal pain between the two groups(P>0.05),there was no significant difference in postoperative bleeding,urinary retention,anal stricture and defecation frequency between the two groups(P>0.05).Conclusion Endoscopic ligation of hemorrhoids for the treatment of II/III degree internal hemorrhoids has the advantages of less trauma,rapid recovery,low cost and less complications.
