针灸联合运动疗法治疗老年卒中后睡眠障碍疗效观察

Clinical Observation on Acupuncture and Moxibustion Combined with Exercise Therapy for Senile Post Stroke Sleep Disorder

目的:观察针灸联合运动疗法治疗老年卒中后睡眠障碍的临床疗效。方法:选取2021年1月至12月福建省泉州市丰泽区北峰街道社区卫生服务中心中医康复科收治的老年卒中后睡眠障碍患者60例作为研究对象,按照随机数字表法分为观察组和对照组,每组30例。观察组实施针灸联合运动疗法,对照组给予艾司唑仑片口服治疗。观察2组临床疗效,比较2组睡眠质量[匹兹堡睡眠质量指数量表(PSQI)]、生命质量[生命质量量表(SF-36)]及日常生活活动能力[日常生活活动能力量表(ADL)]、运动功能[运动功能评分量表(FMA)]和神经功能[美国国立卫生研究院卒中量表(NIHSS)]情况。结果:治疗后,2组PSQI总分和各维度评分、NIHSS评分较本组治疗前下降,且观察组均低于对照组(P<0.05)。治疗后,2组SF-36各项评分、ADL评分、FMA评分均较本组治疗前升高,且观察组均高于对照组(P<0.05)。观察组总有效率100%(30/30),对照组总有效率86.67%(26/30),观察组总有效率高于对照组(P<0.05)。结论:针灸结合运动疗法治疗老年卒中后睡眠障碍,可以提高患者睡眠质量,改善患者日常生活活动能力、运动功能和神经功能,值得临床推广应用。

Objective:To observe the clinical effect of acupuncture and moxibustion combined with kinesitherapy on sleep disorder after stroke in the elderly.Methods:Sixty patients with post-stroke sleep disorders admitted to the Traditional Chinese Medicine Rehabilitation Department of Beifeng Street Community Health Service Center in Fengze District,Quanzhou City,Fujian Province from January to December 2021 were selected as the study subjects.They were randomly divided into an observation group and a control group,with 30 patients in each group.The observation group was given acupuncture and moxibustion+exercise therapy,and the control group was given estazolam orally.Observe the clinical efficacy of two groups and compare their sleep quality(Pittsburgh Sleep Quality Index(PSQI)),quality of life(SF-36),and activities of daily living(ADL),motor function(FMA),and neurological function(NIHSS).Results:After treatment,the total score of PSQI and various dimensions,as well as the NIHSS score of the two groups decreased compared to before treatment,and the observation group was lower than the control group(P<0.05).After treatment,the SF-36 scores,ADL scores,and FMA scores in both groups increased compared to before treatment,and the observation group was higher than the control group(P<0.05).Conclusion:The combination of acupuncture and moxibustion and kinesitherapy in the treatment of sleep disorder after stroke in the elderly can improve the sleep quality of patients,improve their activities of daily living,motor function and neurological function,which is worthy of clinical application.
