家庭功能在前交叉韧带损伤患者心理困扰与应对方式的中介效应

The mediating effect of family function on psychological distress and coping style in patients with anterior cruciate ligament injury

目的 探讨家庭功能在前交叉韧带损伤患者心理困扰与应对方式的中介效应。方法 选择2020年6至2022年6月首都医科大学附属北京积水潭医院收治的234例前交叉韧带损伤患者,采用家庭关怀度指数问卷(APGAR)评估患者家庭功能,凯斯勒心理困扰量表(K10)评估患者心理困扰程度,简易应对方式问卷(simplified coping style questionnaire, SCSQ)评估患者应对方式。分析家庭功能在前交叉韧带损伤患者心理困扰和应对方式之间的中介效应。结果 共发放234份问卷,有效回收230份,有效回收率为98.29%。前交叉韧带损伤患者APGAR评分与心理困扰评分、SCSQ消极应对评分呈负相关(r=-0.513、-0.395,P<0.05),与SCSQ积极应对评分呈正相关(r=0.432,P<0.05),心理困扰评分与SCSQ消极应对评分呈正相关(r=0.407,P<0.05),与SCSQ积极应对评分呈负相关(r=-0.396,P<0.05)。心理困扰评分对SCSQ积极应对评分具有负向预测作用(β=-0.511),对SCSQ消极应对评分具有正向预测作用(β=0.435),增加中介变量APGAR评分,心理困扰评分对SCSQ消极应对评分、SCSQ积极应对评分的效应减弱(β由-0.511、0.435变为-0.306、0.311),提示家庭功能在心理困扰和应对方式之间起部分中介效应。结论 家庭功能部分中介前交叉韧带损伤患者心理困扰和应对方式,建立良好的家庭功能有助于改善心理困扰和应对方式。

Objective To evaluate the mediating effect of family function on psychological distress and coping style in patients with anterior cruciate ligament injury.Methods A total of 234 patients with ACL injury admitted of Beijing Jishuitan Hospital,Capital Medical University,from June 2020 to June 2022 were selected.Family Caring Index(APGAR)was used to assess family function and Kessler Psychological Distress Scale(K10)was used to assess the degree of psychological distress.Simplified coping style questionnaire(SCSQ)was used to evaluate the coping styles of patients.The mediating effect of family function on psychological distress and coping style in patients with ACL injury.Results A total of 234 questionnaires were sent out and 230 were effectively collected,the effective recovery was 98.29%.APGAR score of patients with ACL injury was negatively correlated with psychological distress score and SCSQ negative coping score(r=-0.513,-0.395,P<0.05),positively correlated with SCSQ positive coping score(r=0.432,P<0.05),psychological distress score was positively correlated with SCSQ negative coping score(r=0.407,P<0.05),and negatively correlated with SCSQ positive coping score(r=-0.396,P<0.05).Psychological distress score had a negative predictive effect on SCSQ positive coping score(β=-0.511),and a positive predictive effect on SCSQ negative coping score(β=0.435).When APGAR score was increased,the effect of psychological distress score on SCSQ negative coping score and SCSQ positive coping score decreased(βchanged from-0.511 and 0.435 to-0.306 and 0.311),suggesting that family function had a partial mediating effect between psychological distress and coping style.Conclusion Family function partially mediates psychological distress and coping style in patients with ACL injury.Establishing good family function is helpful to improve psychological distress and coping style.
