热敏灸联合旋转复位手法治疗颈椎病疗效及细胞自噬机制

Mechanism of Heat-Sensitive Moxibustion Combined with New Fixed-Point Rotary Reduction Manipulation in Treatment of Cervical Spondylosis and Autophagy

目的研究新型定点旋转复位手法配合热敏灸治疗颈椎病与细胞自噬的相关机制。方法抽选2020年1月—2022年1月收治的颈椎病患者82例开展研究,按照随机数字表法分为研究组和对照组,各41例,均接受常规治疗,对照组采用常规推拿治疗,研究组在对照组基础上加用新型定点旋转复位手法联合热敏灸治疗;治疗3个疗程后比较中医证候积分疗效、颈椎功能以及自噬调控蛋白表达水平。结果治疗3个疗程后,研究组治疗总有效率为90.24%(37/41),明显高于对照组的70.73%(29/41),P<0.05,差异有统计学意义;研究组中医证候评分颈部疼痛、肩部疼痛、上肢疼痛、上肢麻木各项分值低于对照组,VAS评分、NDI评分低于对照组,Beclin-1水平高于对照组,Bcl-2水平低于对照组,P<0.05,差异有统计学意义。结论新型定点旋转复位手法配合热敏灸治疗颈椎病疗效显著,可在短时间内改善症状,缓解疼痛,并调节细胞自噬水平以恢复颈椎功能,促进预后。

Objective To study the mechanism of the new fixed-point rotary reduction manipulation combined with heat-sensitive moxibustion in the treatment of cervical spondylosis and autophagy.Methods A total of 82 patients with cervical spondylosis who were treated from January 2020 to January 2022 were selected for the study.According to the random number table method,they were divided into a study group and a control group,with 41 cases in each group.All patients received conventional treatment,and the control group received conventional Tuina.On the basis of the control group,the study group was treated with a new fixed-point rotary reduction manipulation combined with heat-sensitive moxibustion.After treatment,the efficacy of TCM syndrome scores,cervical spine function and expression levels of autophagy regulatory proteins were compared.Results After 3 courses of treatment,the total effective rate of the study group was 90.24%(37/41),which was significantly higher than 70.73%(29/41)of the control group(P<0.05),and the difference was statistically significant.The TCM syndrome scores(neck pain,shoulder pain,upper limb pain and upper limb numbness)of the study group were were lower than those of the control group,the VAS score and NDI score were lower than those of the control group,the level of Beclin-1 was higher than that of the control group,and the level of Bcl-2 was lower than that of the control group(P<0.05),and the difference was statistically significant(P<0.05).Conclusion The new fixed-point rotary reduction manipulation combined with heat-sensitive moxibustion has a significant effect in the treatment of cervical spondylosis.It can improve symptoms and relieve pain in a short time,and regulate the level of autophagy to restore cervical function and improve prognosis.
