经椎间孔入路与经椎板间入路内窥镜下椎间盘切除术治疗腰椎间盘突出的效果比较

Comparison of therapeutic effect of percutaneous endoscopic transforaminal discectomy and percutaneous endoscopic interlaminar discectomy on lumbar disc herniation

目的比较经椎间孔入路内窥镜下椎间盘切除术(PETD)及经椎板间入路内窥镜下椎间盘切除术(PEID)治疗腰椎间盘突出(LDH)的效果。方法抽取2022年1月至2023年1月河南省人民医院收治的76例LDH患者,根据手术方法分为两组,38例行PETD治疗者为甲组,38例行PEID治疗者为乙组。比较两组患者的围术期指标、临床疗效及腰椎功能恢复情况。结果乙组患者的手术操作时间短于甲组,术中X线的放射次数少于甲组(P<0.05);治疗后,甲组优良率(86.84%,33/38)与乙组(89.47%,34/38)比较差异未见统计学意义(P>0.05);与治疗前比较,术后6个月时两组患者的日本骨科协会评分均升高,腰椎Oswestry功能障碍指数均降低(P均<0.05),但两组比较差异未见统计学意义(P>0.05)。结论PETD、PEID在LDH的治疗中,临床疗效、腰椎功能恢复情况效果相当,但PEID术能缩短手术操作时间,减少术中X线的放射次数。

ObjectiveTo compare the effects of percutaneous endoscopic transforaminal discectomy(PETD)and percutaneous endoscopic interlaminar discectomy(PEID)in the treatment of lumbar disc herniation(LDH).MethodsSeventy-six patients with LDH admitted to Henan Provincial People’s Hospital from January 2022 to January 2023 were selected and divided into two groups according to surgical methods;of them,38 cases treated by PETD were enrolled into group A,and 38 cases treated by PEID were enrolled into group B.The perioperative indexes,clinical efficacy and lumbar function recovery of the two groups were compared.ResultsThe operation time of the group B was shorter than that of the group A,and the frequency of intraoperative X-ray radiation was less than that of the group A(P<0.05).There was no significant difference in the excellent and good rate between the group A(86.84%,33/38)and the group B(89.47%,34/38),P>0.05.The Japanese Orthopedic Association scores increased,and Oswestry Disability Index decreased in both groups 6 months after surgery,compared with preoperative indexes(all P<0.05);but there was no statistical significance between the two groups(P>0.05).ConclusionsPETD and PEID have similar clinical efficacy and lumbar functional recovery in the treatment of LDH;however,PEID can shorten the surgical operation time,and reduce the number of intraoperative X-ray radiation.
