太原市社区卫生服务机构中医药发展现状调研

Investigation on the Development Status of Traditional Chinese Medicine in Community Health Service Institutions in Taiyuan City

目的调查太原市社区卫生服务机构中医药发展现状,分析存在问题,提出对策建议。方法于2021年10月-2022年4月对太原市6个城区103家社区卫生服务机构负责人进行问卷调查,对6个城区12名社区卫生服务机构负责人或中医科负责人进行半结构访谈。结果101家社区卫生服务机构单独设有中医科,33家设有中医特色专科,19家有中医药发展专项经费,79家支持医保支付。166名中医类别全科医师学历以专科为主,年龄以51岁~60岁为主,工作年限以≥20 a为主,职称以中级和初级为主。社区卫生服务机构虽不同程度地开展了中医适宜技术,但部分项目开展率远低于10%;开展慢性支气管肺炎、恶性肿瘤、老年痴呆中医慢病管理项目的社区卫生服务机构占比远低于50%;开展常见传染病预防的社区卫生服务机构未达100%。86家社区卫生服务机构在新冠疫情期间利用中医药进行宣传教育。社区卫生服务机构中医药发展存在经费投入不足、中医药服务能力有限、中医高素质人才短缺、居民认知与需求度不高等问题。结论太原市社区卫生服务机构中医药服务能力逐渐提高,但仍需要增加财政经费投入,完善医保支付体系,提升医师整体素质,拓宽中医药服务范围,加大中医药宣传力度等。

Objective To investigate the development status of traditional Chinese medicine(TCM)in community health service institutions in Taiyuan city,analyze the existing problems,and put forward countermeasures and suggestions.Methods From October 2021 to April 2022,a questionnaire survey was conducted among the heads of 103 community health service institutions in 6 urban areas of Taiyuan City,and semi-structured interviews were conducted with 12 directors of community health service institutions or heads of TCM Departments in 6 urban areas.Results 101 community health service institutions had their own TCM Departments,33 had TCM specialties,19 had special funds for the development of TCM,and 79 supported medical insurance payments.Among the 166 TCM general practitioners,most of them had specialist education,were aged 51~60,had more than 20 years of working life,and had intermediate and primary professional titles.Although the community health service institutions carried out the appropriate technology of TCM to different degrees,the implementation rate of some projects was far less than 10%.The number of community health service institutions carrying out TCM chronic disease management projects for chronic bronchopneumonia,malignant tumor and senile dementia was far less than 50%.Less than 100%of community health service institutions had carried out the prevention of common infectious diseases.86 community health service institutions used TCM for publicity and education during the epidemic.There were some problems in the development of TCM in community health service institutions,such as insufficient funding,limited service capacity,shortage of high-quality talents ofTCM,and low awareness and demand of residents.Conclusion The TCM service ability of community health service institutions is gradually improved in Taiyuan city,but it is still necessary to increase financial input,improve the medical insurance payment system,improve the overall quality of doctors,expand the scope of TCM service,and strengthen the publicity of TCM.
