香砂六君子汤治疗脾虚湿盛型宫颈HPV感染的疗效观察及对免疫功能的影响

Effffect of Xiangsha Liujunzi Decoction on cervical HPV infection with syndrome of spleen defificiency and dampness excess and its inflfluence on immune function

目的观察香砂六君子汤治疗脾虚湿盛型宫颈人乳头瘤病毒(HPV)感染的疗效及对免疫功能的影响。方法选择2019年1月-2022年3月医院收治的92例脾虚湿盛型宫颈HPV感染患者,以随机数字表法将其分为研究组(n=46)与对照组(n=46)。对照组接受常规重组人干扰素α-2b治疗,研究组在对照组的基础上联合香砂六君子汤治疗。对比2组中医证候评分、临床疗效、HPV病毒载量、HPV转阴率、白细胞介素-17(IL-17),IL-2、肿瘤坏死因子α(TNF-α)、免疫功能及不良反应。结果治疗后,2组主症评分、次症评分、总评分均降低(P<0.05),且研究组更低(P<0.05)。研究组总有效率(93.48%)比对照组(78.26%)高(P<0.05)。治疗后,2组HPV病毒载量均降低(P<0.05),且研究组更低(P<0.05);研究组HPV转阴率(91.30%)比对照组(76.09%)高(P<0.05)。治疗后,2组血清IL-17、IL-2、TNF-α水平均降低(P<0.05),且研究组更低(P<0.05)。治疗后,2组CD3+、CD4+水平均升高(P<0.05),且研究组更高(P<0.05);治疗后,2组CD8+水平均降低(P<0.05),且研究组更低(P<0.05)。2组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论香砂六君子汤治疗脾虚湿盛型宫颈HPV感染疗效确切,可提高HPV转阴率,减轻患者炎症反应,改善免疫功能,且安全可靠。

Objective To observe the curative effect of Xiangsha Liujunzi Decoction on cervical human papillomavirus(HPV)infection with the syndrome of spleen deficiency and dampness excess and its influence on immune function.Methods Ninety-two patients with cervical HPV infection with the pattern of spleen deficiency and dampness excess admitted to our hospital from January 2019 to March 2022 were selected and divided into a study group(n=46)and a control group(n=46)by random number table method.The control group was treated with conventional recombinant human interferonα-2b,while the study group was treated with Xiangsha Liujunzi Decoction on the basis of the control group.The TCM syndrome score,clinical efficacy,HPV viral load,HPV negative conversion rate,interleukin-17(IL-17),IL-2,tumor necrosis factor-α(TNF-α),immune function and adverse reactions were compared between the two groups.Results After treatment,the main symptom score,secondary symptom score and total score were decreased in the two groups(P<0.05),and the study group was much lower(P<0.05).The total effective rate of the study group(93.48%)was higher than that of the control group(78.26%)(P<0.05).After treatment,the HPV viral load was decreased in both groups(P<0.05),and it was much lower in the study group(P<0.05).The negative conversion rate of HPV in the study group(91.30%)was higher than that in the control group(76.09%)(P<0.05).After treatment,the serum levels of IL-17,IL-2 and TNF-αwere decreased in both groups(P<0.05),and they were much lower in the study group(P<0.05).After treatment,the levels of CD3+and CD4+were increased in the two groups(P<0.05),and they were much higher in the study group(P<0.05).After treatment,the level of CD8+was decreased in both groups(P<0.05),and it was even lower in the study group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Xiangsha Liujunzi Decoction is effective in the treatment of cervical HPV infection with the pattern of spleen deficiency and dampness excess.It can improve its negative conversion rate,reduce the inflammatory response of patients,improve immune function,and is safe and reliable.
