应用脑CT灌注成像评估丁苯酞对急性缺血性脑血管疾病治疗前、后侧枝循环影响

摘要 目的应用脑CT灌注成像评估丁苯酞对急性缺血性脑血管疾病治疗前、后侧枝循环影响;方法选取2020年6月-2022年3月收治的98例急性缺血性脑血管病患者分为对照组和治疗组,对照组给予常规治疗,治疗组在对照组基础上联合丁苯酞治疗,两组均给予脑CT灌注成像评估,比较两组临床疗效,治疗前后美国国立卫生院卒中量表评分(NIHSS)、修正Rankin量表(mRS)评分、患侧脑灌注参数图包括脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)和对比剂峰值时间(TTP)。结果治疗组总有效率显著高于对照组(χ^(2)=4.346,P<0.05)。治疗前两组NIHSS、mRS评分无显著性差异(P>0.05),治疗后治疗组NIHSS、mRS评分显著低于对照组(P<0.05)。治疗前两组患侧CBF、CBV、MTT、TTP无显著差异(P>0.05),治疗后两组CBV、CBF均显著提高,MTT、TTP均显著降低,且治疗后治疗组CBV、CBF高于对照组,MTT、TTP低于对照组(P<0.05)。结论应用脑灌注CT成像技术评估丁苯酞治疗急性缺血性脑血管疾病临床疗效是可行的,丁苯酞可有效提高急性缺血性脑血管疾病的临床疗效,改善侧枝循环,缓解临床症状,值得推广。 Objective To evaluate the effects of butylphthalein on anterior and posterior collateral circulation of acute ischemic cerebrovascular diseases by cerebral CT perfusion imaging.Methods Ninety-eight patients with acute ischemic cerebrovascular disease admitted from June 2020 to March 2022 were divided into control group and treatment group.The control group received conventional treatment,while the treatment group was treated with butylphthalide.Both groups were evaluated by cerebral CT perfusion imaging.NIHSS,mRS,CBF,CBV,MTT,TTP parameters before and after treatment.Results The total effective rate of clinical efficacy in the treatment group was 87.76%,which was significantly higher than that in the control group(77.55%)(χ^(2)=4.346,P<0.05).NIHSS and mRS Scores of the two groups had no significant difference before treatment(P>0.05),and NIHSS and mRS Scores of the two groups were significantly decreased after treatment.After treatment,NIHSS and mRS Scores in the treatment group were significantly lower than those in the control group(P<0.05),and CBF,CBV,MTT and TTP parameters in the affected side were not significantly different between the two groups before treatment(P>0.05).After treatment,CBV and CBF parameters in the two groups were significantly increased,while MTT and TTP parameters were significantly decreased.After treatment,CBV and CBF were increased in the treatment group,while MTT and TTP parameters were decreased more significantly(P<0.05).Conclusion The application of cerebral perfusion CT imaging technology to evaluate the clinical efficacy of butylphthalein in the treatment of acute ischemic cerebrovascular diseases is feasible,and butylphthalein can effectively improve the clinical efficacy of acute ischemic cerebrovascular diseases,improve collateral circulation,relieve clinical symptoms,so it is recommended to popularize.
