摘要
目的探讨1例重症肺炎患者利用肺泡灌洗技术查找致病菌,为抗感染方案制定提供用药依据。方法临床药师对1例不明原因反复发热患者治疗过程进行全程用药分析。结果患者在使用多种广谱抗菌药物情况下仍反复发热,通过肺泡灌洗技术查找出致病菌为铜绿假单胞菌,为抗感染“降阶梯”治疗提供有力支撑;同时通过肺泡灌洗冲洗出大量脓液,起到感染灶清除的作用。结论对于不明原因反复发热的重症肺炎患者,充分利用肺泡灌洗技术除了能帮助查找致病菌为合理用药提供用药依据,同时肺泡灌洗技术还能起到清除肺部病灶的作用。
OBJECTIVE To investigate a case of severe pneumonia using alveolar lavage technology to find pathogens and provide medication basis for the formulation of anti-infective programs.METHODS The clinical pharmacist analyzed the medications of a patient with unexplained recurrent fever during the course of treatment.RESULTS The patient still had repeated fever despite using a variety of broad-spectrum antibacterial drugs.The alveolar lavage technique was used to find the pathogenic bacteria as Pseudomonas aeruginosa,which provides strong support for the anti-infective“de-escalation”treatment;at the same time,the alveolar lavage was used to flush a large amount of pus,which plays a role in clearing the infection.CONCLUSION For patients with severe pneumonia with repeated fever of unknown cause,the full use of alveolar lavage technology can not only help find pathogenic bacteria and provide medication basis for rational use of drugs,but also can remove lung lesions.
