基于数据挖掘技术探讨补益类中药治疗早发性卵巢功能不全的用药规律

Study on the medication rules of traditional Chinese medicine in the treatment of Premature Ovarian Insufficiency based on data mining technology

目的利用数据挖掘技术,依托中医传承辅助平台系统(V2.5),探讨补益类中药治疗早发性卵巢功能不全的用药规律。方法通过检索中文数据库中2000年到2022年采用中药或中西医结合治疗早发性卵巢功能不全的临床研究类文献,筛选并提取文献数据,构建数据库,通过中医传承辅助平台系统(V2.5)进行数据分析。结果共纳入期刊文献139篇,处方152首,涉及中药134种;药物使用总频次1770次,其中排名前5位的高频中药分别为熟地黄、当归、菟丝子、甘草和山药;药物四气以温、平、寒为主;五味以甘、辛、苦为主;药物归经以肝、肾、脾和心经居多;用药多以补虚药、活血化瘀药和清热药为主;涉及中医证型17种,其中排名前5位为肾虚肝郁证、肝肾阴虚证、肾虚证、肾虚血瘀证和脾肾阳虚证。在关联规则下进行组方规律分析,核心药物组合为“熟地-菟丝子-当归”;进行复杂系统熵层次聚类分析,得出核心药物组合14个(7组),在此基础上采用无监督的熵层次聚类分析,获得潜在新处方7个。新方1:肉苁蓉-桂枝-山药-茯苓-紫石英;新方2:仙茅-肉桂-附子-淫羊藿-杜仲-白术;新方3:覆盆子-桑椹-沙苑子-当归;新方4:桃仁-鹿角胶-补骨脂-山茱萸-龟甲胶;新方5:菟丝子-北沙参-枸杞子-川芎-补骨脂;新方6:菟丝子-酸枣仁-麦冬-阿胶;新方7:黄柏-知母-玄参-泽泻。结论通过数据发掘发现补益类中药治疗早发性卵巢功能不全,多以补肾填精、益气活血、疏肝理气、调经助孕为主要治疗原则,并佐以温阳之品,其用药原则可为当前中医药治疗早发性卵巢功能不全及新药开发提供理论依据。

Objective The medication rules of traditional Chinese medicine in the treatment of Premature Ovarian Insufficiency were explored by data mining technology and relying on the traditional Chinese medicine inheritance auxiliary platform system(V2.5).Methods By searching the clinical research literatures of traditional Chinese medicine or integrated traditional Chinese and western medicine in the treatment of ovarian dysfunction from 2000 to 2022 in the Chinese database,the literature data were screened and extracted to construct a database,and the data were analyzed by the Traditional Chinese Medicine inheritance auxiliary platform system(V2.5).Results A total of 139 journals articles were included,with 152 prescriptions,involving 134 kinds of traditional Chinese medicine.The total frequency of drug use was 1770 times,and the top 5 high-frequency Chinese herbs were Rehmanniae radix,Angelica sinensis,Cuscutae,Licorice and yam.The four qi of medicine were mainly warm,flat and cold.The five tastes were mainly sweet,spicy and bitter.The majority of drugs belonged to liver,kidney,spleen and heart meridians.The main drugs were tonifying deficiency,promoting blood circulation and removing blood stasis,and clearing heat.There were 17 TCM syndromes involved,among which the top 5 syndromes were kidney deficiency and liver stagnation syndrome,liver and kidney Yin deficiency syndrome,kidney deficiency syndrome,kidney deficiency and blood stasis syndrome,spleen and kidney Yang deficiency syndrome.According to the analysis of the prescription rule under the association rules,the core drug combination was"Shudi-Cuscutazi-Angelica sinensis".We found 14 potential core drug combinations(7 groups),and 7 potential new prescriptions were obtained based on the finding through complex hierarchical clustering analysis.New prescription 1:Cistanche deserticola,Cassia twig,Yam and Poria poria amethyst;New prescription 2:Rhizoma curculipa,cinnamon,Aconite,Epimedium,Eucommia,Atractylodes;New recipe 3:raspberry mulberry,Sha Yuan and Angelica sinensis;New formula 4:peach kernel antler glue psoraleae_dogwood tortoise shell glue;New formula 5:Cuscutazi north Radix salvia miltiorrhiza_Wolfberry Chuanxiong_Psoraleae;New formula 6:Cuscutazi sour jujube kernel maidong_ejiao;New prescription 7:Huangbai Anemarrhizae rhizoma and Rhizoma scrophulariae rhizoma.Conclusion Through data mining,the medication characteristics and prescription rules of traditional Chinese medicine in the treatment Premature Ovarian Insufficiency mainly focuses on tonifying the kidney and filling the essence,supplementing qi and promoting blood circulation,soothing the liver and regulating qi,regulating menstruation and assisting pregnancy,and is supplemented with warm yang products.The medication principles can provide a theoretical basis for the current treatment of early-onset ovarian dysfunction with traditional Chinese medicine and the development of new drugs.
