冠心宁片治疗颈动脉粥样硬化的疗效观察

摘要 目的观察冠心宁片对颈动脉粥样硬化(CAS)患者的血脂水平、颈动脉斑块及炎症指标的影响。方法选取2020年4月至2022年1月符合纳入标准的CAS患者180例,按照随机数表法分为观察组与对照组,各90例。两组均予辛伐他汀片20 mg治疗,观察组在此基础上加用冠心宁片。两组患者干预治疗12周后,比较血脂水平、颈动脉斑块、血清炎症指标。结果治疗12周后,两组患者TC、TG、LDL-C比治疗前明显降低(P<0.05),且观察组降低幅度大于对照组(P<0.05)。两组患者颈动脉内膜-中层厚度(IMT)、最大斑块面积均较前明显缩小(P<0.05),且观察组优于对照组(P<0.05)。两组患者治疗后炎症指标MLR、NLR水平低于治疗前(P<0.05),且观察组降低幅度大于对照组(P<0.05)。结论冠心宁片可有效改善CAS,其机制可能与减轻局部炎症反应有关。冠心宁片可降低基础治疗导致HDL-C过高产生骨质疏松、CAS加剧的风险。 Objective To observe the effect of Guanxinning tablet on serum lipid level,carotid plaque and serum inflammation in patients with carotid atherosclerosis(CAS).Methods 180 outpatient and inpatient patients met the inclusion criteria from April 2020 to January 2020,and 90 cases were equally divided between the observation group and the control group according to the random number table.In both groups,20mg simvastatin tablets were given conventional treatment,and the observation group added guanxinning tablets on this basis.Comparanalysis of the changes of serum lipid level,carotid plaque and serum inflammation indexes after 12 weeks of intervention.Results After 12 weeks of treatment,total cholesterol(TC),glycerin trilaurate(TG),and low density lipoprotein cholesterol(LDL-C)in both groups of patients were significantly reduced compared to before treatment(P<0.05),and the observed group had a greater reduction than the control group(P<0.05).The carotid intima-media thickness(IMT)and maximum plaque area of both groups of patients were significantly reduced compared to before(P<0.05),and the observation group was better than the control group(P<0.05).After treatment,the levels of inflammatory markers that ratio of monocyte count to lymphocyte count(MLR)and ratio of neutrophil count to lymphocyte count(NLR)in both groups of patients were lower than before treatment(P<0.05),and the decrease in the observation group was greater than that in the control group(P<0.05).Conclusion It can effectively improve carotid atherosclerosis,and its mechanism may be related to the reduction of local inflammatory response.The addition of guanxinning tablets can reduce the risk of osteoporosis and carotid atherosclerosis caused to high density lipoprotein cholesterol(HDL-C).
