炔诺酮片与黄体酮胶囊治疗围绝经期异常子宫出血患者的效果比较

Comparison of effects of Norethisterone tablets and Progesterone capsules in treatment of patients perimenopausal with abnormal uterine bleeding

目的:比较炔诺酮片与黄体酮胶囊治疗围绝经期异常子宫出血(AUB)患者的效果。方法:选取2019年1月至2022年10月该院收治的192例围绝经期AUB患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各96例。对照组采用黄体酮胶囊治疗,观察组采用炔诺酮片治疗,比较两组临床疗效、治疗前后子宫内膜厚度、血清性激素[雌二醇、促黄体生成素(LH)、促卵泡生成素(FSH)]水平、血红蛋白水平、血管紧张素-2(Ang-2)水平和血管内皮生长因子(VEGF)水平。结果:观察组治疗总有效率为95.83%,高于对照组的83.33%,差异有统计学意义(P<0.05);治疗后,两组血红蛋白水平高于治疗前,且观察组高于对照组,两组子宫内膜厚度、雌二醇、LH、FSH、Ang-2、VEGF水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:炔诺酮片治疗围绝经期AUB患者可提高治疗总有效率和血红蛋白水平,降低子宫内膜厚度以及血清雌二醇、LH、FSH、Ang-2、VEGF水平,效果优于黄体酮胶囊治疗。

Objective:To compare effects of Norethisterone tablets and Progesterone capsules in treatment of patients perimenopausal with abnormal uterine bleeding(AUB).Methods:A prospective study was conducted on 192 patients with perimenopausal AUB admitted to the hospital from January 2019 to October 2022.They were divided into observation group and control group according to the random number table method,96 cases in each group.The control group was treated with Progesterone capsules,while the observation group was treated with Norethisterone tablets.The clinical efficacy,the endometrial thickness,the serum sex hormones[estradiol,luteinizing hormone(LH),follicle stimulating hormone(FSH)]levels,the hemoglobin level,the angiotensin-2(Ang-2)level,and the vascular endothelial growth factor(VEGF)level were compared between the two groups.Results:The total effective rate of the observation group was 95.83%,which was significantly higher than 83.33%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the hemoglobin levels of the two groups were higher than those before the treatment,and that in the observation group was higher than that in the control group;the endometrial thickness and the estradiol,LH,FSH,Ang-2 and VEGF levels in the two groups were lower than those before the treatment,and those in the observation group were lower than those in the control group;and the difference was statistically significant(P<0.05).Conclusions:Norethisterone tablets in the treatment of the perimenopausal AUB patients can improve the total effective rate of treatment and the hemoglobin levels and reduce the endometrial thickness and the serum estradiol,LH,FSH,Ang-2 and VEGF levels.Moreover,it is superior to Progesterone capsules treatment.
