热敏灸联合中药熏洗治疗寒湿痹阻型肩周炎患者的临床效果

Clinical effect of heat-sensitive moxibustion combined with traditional Chinese medicine fumigation in patients with cold-dampness impediment type shoulder periarthritis

目的探讨热敏灸联合中药熏洗在寒湿痹阻型肩周炎患者中的临床效果。方法选取2020年6月至2022年6月九江市中医医院收治的98例肩周炎患者作为研究对象,按照随机数字表法分为对照组(49例)及观察组(49例)。对照组予以常规西药治疗,观察组加用热敏灸联合中药熏洗治疗,持续治疗4周。比较两组患者临床疗效、肩关节功能、疼痛评分、肩关节活动度、日常生活能力、炎症因子及安全性。结果观察组总有效率高于对照组,治疗后Constant-Murley肩关节评分中疼痛、日常生活活动、主动活动范围、肌力及总分高于对照组,差异有统计学意义(P<0.05);观察组治疗后视觉模拟评分法(VAS)评分低于对照组,前屈活动度、外展活动度、后伸活动度、Barthel量表(BI)评分高于对照组,差异有统计学意义(P<0.05);观察组治疗后白介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平低于对照组,差异有统计学意义(P<0.05);两组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。结论热敏灸联合中药熏洗可提高寒湿痹阻型肩周炎患者疗效,减轻炎症损伤,增加肩关节活动度,改善日常生活能力,安全可靠。

Objective To investigate the clinical effect of heat-sensitive moxibustion combined with traditional Chinese medicine fumigation in patients with cold-dampness impediment type shoulder periarthritis.Methods A total of 98 patients with shoulder periarthritis treated in Jiujiang Hospital of Traditional Chinese Medicine from June 2020 to June 2022 were selected as the study objects,and they were divided into control group(49 cases)and observation group(49 cases)according to random number table method.The control group was treated with conventional Western medicine,and the observation group was treated with heat-sensitive moxibustion combined with traditional Chinese medicine fumigation for 4 weeks.The clinical efficacy,shoulder joint function,pain score,shoulder joint range of motion,ability of daily living,inflammatory factors and safety were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the scores of Constant-Murley shoulder joint pain,activities of daily living,active range of motion,muscle strength and total score after treatment were higher than those of the control group,with statistical significances(P<0.05).The visual analogue scale(VAS)score of the observation group was lower than that of the control group after treatment,and the scores of anterior flexion,abductive,postextension and Barthel index(BI)were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Heat-sensitive moxibustion combined with traditional Chinese medicine fumigation can improve the curative effect,reduce inflammation and injury,increase the mobility of shoulder joint,improve the ability of daily life,and be safe and reliable.
