无托槽隐形矫治器对口腔正畸患者龈沟液中炎症因子水平及牙周状况的影响

Effects of Bracketless Concealed Appliance on the Levels of Inflammatory Factors in Gingival Crevicular Fluid and Periodontal Status in Orthodontic Patients

目的:探究无托槽隐形矫治器对口腔正畸患者龈沟液中炎症因子水平及牙周状况的影响。方法:按随机数字表法将新余市妇幼保健院2021年1月—2022年1月收治的80例口腔正畸患者分为两组,各40例。对照组予以传统托槽矫治器治疗,研究组予以无托槽隐形矫治器治疗。对比两组临床疗效、倾斜牙矫正时间、转位牙扭正时间、完成治疗时间、可溶性细胞间黏附分子-1(sICAM-1)、氧化应激指标[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]、炎症因子[基质金属蛋白酶-8(MMP-8)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)]、牙周指标[牙龈指数(GI)、牙菌斑指数(PLI)、牙周袋深度(PD)、龈沟出血指数(SBI)]和不良反应发生情况。结果:研究组治疗后总有效率高于对照组(P<0.05);研究组倾斜牙矫正时间、转位牙扭正时间、完成治疗时间均短于对照组(P<0.05);研究组治疗6个月后s ICAM-1、MMP-8、IL-1β、TNF-α水平均低于对照组,SOD、GSH-Px水平均高于对照组(P<0.05);研究组治疗6个月后GI、PLI、SBI评分和PD均低于对照组(P<0.05);研究组治疗后不良反应评分均低于对照组(P<0.05)。结论:口腔正畸患者采用无托槽隐形矫正器治疗效果确切,可有效抑制牙周炎症反应,改善牙周指数,有利于缩短治疗时间,且不良反应较轻。

Objective:To investigate the effect of bracketless concealed appliance on the levels of inflammatory factors in gingival crevicular fluid and periodontal status in orthodontic patients.Method:According to the method of random number table,80 orthodontic patients admitted to Xinyu Maternal and Child Health Hospital from January 2021 to January 2022 were divided into two groups,40 cases each.The control group was treated with traditional bracket appliance,while the study group was treated with bracketless concealed appliance.The clinical efficacy,correction time of oblique teeth,correction time of transposition teeth,completion time of treatment,soluble intercellular adhesion molecule-1(sICAM-1),oxidative stress indexes[superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)],inflammatory factors[matrix metalloproteinase-8(MMP-8),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)],periodontal indexes[gingival index(GI),plaque index(PLI),periodontal pocket depth(PD),sulcus bleeding index(SBI)]and adverse reactions were compared between the two groups.Result:The total effective rate of the study group after treatment was higher than that of the control group(P<0.05);the correction time of tilted teeth,the twisting time of transposed teeth and the completion time of treatment in the study group were shorter than those in the control group(P<0.05);after 6 months of treatment,the levels of sICAM-1,MMP-8,IL-1βand TNF-αin the study group were lower than those in the control group,while the levels of SOD and GSH-Px were higher than those in the control group(P<0.05);after 6 months of treatment,GI,PLI and SBI scores and PD of the study group were lower than those of the control group(P<0.05);the adverse reaction scores of the study group were lower than those of the control group(P<0.05).Conclusion:The treatment of orthodontic patients with bracketless concealed appliance is effective,it can effectively inhibit the periodontal inflammatory reaction,improve the periodontal index,shorten the treatment time,and has less adverse reactions.
