前交叉韧带损伤早期治疗与延期治疗的临床效果比较

Comparison of Clinical Effects Between Early Treatment and Delayed Treatment of Anterior Cruciate Ligament Injury

目的比较前交叉韧带(anterior cruciate ligament,ACL损伤早期治疗与延期治疗的临床效果。方法选取2019年11月—2022年10月在三明市第二医院就诊的60例ACL损伤患者,60例患者均接受自体腘绳肌肌腱移植单束重建术,以治疗时间不同为依据将其分为两组,≤1年者为早期组,>1年者为延期组,各30例。分析两组并发症、膝关节评分、膝关节活动度。结果术后,早期组半月板损伤、髁间窝狭窄、股关节软骨损伤较延期组低(P<0.05);两组负重区软骨损伤比较,差异无统计学意义(P>0.05);术后,早期组视觉模拟评分(visual analogue score,VAS)、利斯霍尔姆(Lysholm)评分、国际膝关节文献委员会膝关节评估表(the International Knee Documentation Committee knee evaluation form,IKDC)评分均较延期组优(P<0.05);术后,早期组伸膝受限度(1.93±0.36)°、屈膝受限度(5.14±1.42)°,低于延期组(2.35±0.48)°、(6.53±1.59)°,早期组膝关节活动度(121.38±6.04)°,高于延期组(116.75±6.82)°(P<0.05)。结论与延期治疗相比,早期治疗ACL损伤疗效更佳,利于膝关节功能提高。

Objective To compare the clinical effect of early treatment and delayed treatment for anterior cruciate ligament(ACL)injury.Methods A total of 60 patients with ACL injury treated in Sanming Second Hospital from November 2019 to October 2022 were selected,60 patients received autologous hamstring tendon transplantation single bundle reconstruction.They were divided into two groups according to different treatment time,those≤1 year as the early group,and those>1 year as the delayed group,with 30 cases each.Complications,knee joint score and knee joint motion were analyzed.Results After surgery,meniscus injury,intercondylar fossa stenosis and femoral joint cartilage injury in early group were lower than those in delayed group(P<0.05).There was no statistical significance in cartilage injury in the weight-bearing area between the two groups(P>0.05).After surgery,visual analogue score(VAS),Lysholm score and the International Knee Documentation Committee knee evaluation form(IKDC)in the early group were better than those in the delayed group(P<0.05).After surgery,the knee extension limited degree(1.93±0.36)°and knee bending limited degree(5.14±1.42)°in the early group were lower than the(2.35±0.48)°and(6.53±1.59)°in the delayed group,and the knee motion in the early group was(121.38±6.04)°,higher than the delay group(116.75±6.82)°(P<0.05).Conclusion Compared with delayed treatment,early treatment of ACL injury has better effect,which is beneficial to improve the function of knee j oint.
