住院药房抗菌药物处方实施处方前置审核系统的效果观察

Observation on the Effect of Implementing Prescription Pre-Review System for Antibacterial Drug Prescriptions in Inpatient Pharmacy

目的观察住院药房抗菌药物处方实施处方前置审核系统的应用效果。方法选取2021年1—12月黄石市第四医院药剂科1000张住院药房抗菌药物处方为对照组,另选取2022年1—12月黄石市第四医院药剂科1000张住院药房抗菌药物处方为研究组。研究组实施处方前置审核系统,对照组未实施处方前置审核系统。比较两组干预后抗菌药物处方合格率、提醒修改率、药师干预率、处方质量(适应证、用法用量、相互作用、药物过敏、特殊人群、配伍禁忌)、不合格处方分类情况(无适应证用药、重复给药、用法不当、超剂量用药、用药途径不当、用药频次不当)。结果干预后,研究组抗菌药物处方合格率、提醒修改率、药师干预率(95.10%、2.80%、2.10%)高于对照组(89.20%、1.10%、0.50%),差异有统计学意义(P<0.05);干预后,研究组适应证、用法用量、相互作用、药物过敏、特殊人群、配伍禁忌评分较对照组偏高,差异有统计学意义(P<0.05);干预后,两组无适应证用药、重复给药、用法不当、超剂量用药、用药途径不当、用药频次不当情况比较,差异无统计学意义(P>0.05)。结论住院药房抗菌药物处方实施处方前置审核系统,可提高抗菌药物处方合格率、提醒修改率、药师干预率及处方质量。

Objective Observation of the application effect of prescription pre review system for antibacterial drugs in inpatient pharmacy.Methods A total of 1000 inpatient pharmacy antibacterial drug prescriptions from the department of pharmacy,Huangshi City Fourth Hospital from January to December 2021 were selected as the control group,and 1000 inpatient pharmacy antibacterial drug prescriptions from the department of pharmacy,Huangshi City Fourth Hospital from January to December 2022 were selected as the research group.The research group implemented a prescription pre audit system,while the control group did not implement a prescription pre audit system.The qualification rate,reminder modification rate,pharmacist intervention rate,prescription quality(indications,usage and dosage,interactions,drug allergies,special populations,compatibility contraindications),and classification of unqualified prescriptions(non indications,repeated administration,improper use,excessive dosage,improper medication route,and improper frequency)between two groups of antibacterial drug prescriptions were compared.Results After intervention,the qualified rate of antibiotics prescription,the reminding modification rate and the intervention rate of pharmacists in the study group(95.10%,2.80%,2.10%)were higher than those in the control group(89.20%,1.10%,0.50%),and the difference was statistically significant(P<0.05).After intervention,the scores of indications,usage and dosage,interaction,drug allergy,special population and incompatibility in the study group were higher than those in the control group,with statistical significance(P<0.05).After intervention,there was no statistically significant difference between the two groups in drug use without indications,repeated administration,improper usage,overdose,improper route of medication and improper frequency of medication(P>0.05).Conclusion Implementing a prescription pre review system for antibacterial drug prescriptions in inpatient pharmacies can improve the qualification rate,reminder modification rate,pharmacist intervention rate,and prescription quality.
