口服匹维溴铵及等渗甘露醇对改善^(18)F-FDG PET/CT肠道成像质量的技术探讨

摘要 目的:探讨口服匹维溴铵及等渗甘露醇对改善^(18)F-氟代脱氧葡萄糖(^(18)F-FDG)正电子发射计算机体层成像(PET/CT)肠道成像质量的影响。方法:将2022年3月至7月行"8F-FDGPET/CT检查的180例患者随机分为3组:对照组(口服饮用水)、试验1组(口服等渗甘露醇)、试验2组(口服匹维溴铵片和等渗甘露醇),每组60例。各组均行PET/CT常规扫描。采用秩和检验比较3组间肠道充盈程度、扩张程度、生理性浓聚程度及总体图像质量的差异,采用χ^(2)检验分析3组间不良反应发生率的差异。结果:试验2组在小肠充盈程度、扩张程度、生理性浓聚程度及总体图像质量明显优于对照组(z=-5.858,z=-4.987,z=-7.343,z=-7.282,均P<0.05),与试验1组差异无统计学意义(z=-1.203,z=-1.533,z=-1.669,z=-1.758,均P>0.05)。在结直肠扩张程度及生理性浓聚程度上,试验2组优于对照组(z=-7.500,z=-5.249,均P<0.05)及试验1组(z=-3.162,z=-3.451,均P<0.05);试验2组结直肠充盈程度优于对照组(z=-9.455,P<0.05),与试验1组差异无统计学意义(z=-0.680,P>0.05)。试验2组结直肠总体图像质量优于对照组与试验1组(z=-8.264,z=-3.130,均P<0.05)。试验1组与试验2组检查后腹泻发生率高于对照组(χ^(2)=12.842,P<0.05),试验1组与试验2组差异无统计学意义(P>0.05)。结论:^(18)F-FDGPET/CT检查前口服匹维溴铵片,并间断饮用等渗甘露醇溶液,能获得较好的肠道充盈及扩张效果,降低肠道放射性摄取程度,提高总体图像质量,有助于病变的判断。 Purpose:To investigate the effect of oral pinaverium bromide tablets and isotonic mannitol on improving the quality of ^(18)F-fluorode-oxyglucose(^(18)F-FDG)positron emission tomography and computed tomography(PET/CT)intestinal imaging.Methods:A total of 180 patients who underwent^(18)F-FDG PET/CT scans between March and July 2022 were randomly assigned to three groups,each of which consisted of 60 patients:the control group(oral water),the test 1 group(oral isotonic mannitol),and the test 2 group(oral pinaverium bromide tablets and isotonic mannitol).Every group had a standard PET/CT scan.The degree of intestinal canal fullness,dilatation,physiological uptake,and overall picture quality were compared across the three groups using the Kruskal-Wallis test and Mann-Whitney test,and the incidence of adverse reactions was examined using the χ^(2)test.Results:In terms of fullness,dilatation,physiological uptake,and overall image quality of the small bowel,the test 2 group significantly outperformed the control group(z=-5.858,z=-4.987,z=-7.343,z=-7.282,all P<0.05),while the test 1 group did not differ statistically from the control group(z=-1.203,z=-1.533,z--1.669,z=-1.758,all P>0.05).In terms of the degree of colorectal dilatation and physiological uptake,the test 2 group was better than the control group(z=-7.500,z=-5.249,both P<0.05)and the test 1 group(z--3.162,z=-3.451,both P<0.05);the degree of colorectal filling in the test 2 group was better than the control group(z=-9.455,P<0.05),and the difference with the test 1 group was with no statistical significance(z=-0.680,P>0.05).The overall image quality of colorectum in test 2 group was better than that in the control and test 1 groups(z=-8.264,z=-3.130,both P<0.05).The incidence of post-examination diarrhea was higher in the test 1 and test 2 groups than that in the control group(χ^(2)=12.842,P<0.05),and the difference between the test 1 and test 2 groups was with no statistical significance(P>0.05).Conclusion:Taken pinaverium bromide tablets and isotonic mannitol solution prior to an F-FDG PET/CT scan will promote intestine fullness and dilatation,reduce intestinal physiological uptake,improve overall image quality,and help in the detectionoflesions.
