超声引导下微创热消融术治疗甲状腺结节的效果分析 被引量:1

摘要 目的分析超声引导下微创热消融术治疗甲状腺结节的效果。方法90例甲状腺结节患者,均实施超声引导下微创热消融术治疗。分析患者临床相关指标、术后并发症发生情况,比较患者治疗前后甲状腺功能指标[游离三碘甲状腺原氨酸(FT3)、促甲状腺激素(TSH)和游离甲状腺素(FT4)]、结节体积及生活质量评分。结果患者手术用时为(35.84±4.27)min,术中出血量为(12.23±3.10)ml,住院时长为(3.44±1.07)d。患者术后发生喉返神经损伤0例、甲状腺功能减退2例、疼痛2例,并发症发生率为4.44%。患者治疗后FT3(3.10±0.09)pmol/L、FT4(12.31±1.00)pmol/L低于治疗前的(5.32±0.56)、(18.45±3.08)pmol/L,TSH(3.12±0.56)μIU/ml高于治疗前的(0.31±0.03)μIU/ml,结节体积(0.32±0.03)cm3小于治疗前的(1.56±0.21)cm3,差异有统计意义(P<0.05)。患者治疗后心理健康评分为(92.41±3.43)分、认知职能评分为(90.27±4.37)分、社会职能评分为(91.23±3.62)分、情绪职能评分为(91.49±4.01)分、角色职能评分为(92.33±3.61)分、生理职能评分为(95.23±3.01)分,均高于治疗前的(78.31±5.21)、(75.43±5.24)、(78.13±5.71)、(78.13±5.71)、(73.09±5.19)、(72.23±5.16)分,差异有统计意义(P<0.05)。结论超声引导下微创热消融术用于甲状腺结节的治疗中,能缩短手术时间与住院时间,降低出血量与并发症发生率,提高患者生活质量。 Objective To analyze the effect of ultrasound-guided minimally invasive thermal ablation in the treatment of thyroid nodules.Methods 90 patients with thyroid nodules were treated with ultrasound-guided minimally invasive thermal ablation.The clinical indicators and postoperative complications were analyzed,and the thyroid function indicators[free triiodothyronine(FT3),thyroid stimulating hormone(TSH)and free thyroxine(FT4)],nodule volume and quality of life scores were compared before and after treatment.Results The operative time was(35.84±4.27)min,the amount of intraoperative blood loss was(12.23±3.10)ml,and the length of hospital stay was(3.44±1.07)d.There were 0 case of recurrent laryngeal nerve injury,2 cases of hypothyroidism,and 2 cases of pain after surgery,with a complication rate of 4.44%.After treatment,FT3 of(3.10±0.09)pmol/L and FT4 of(12.31±1.00)pmol/L were lower than those of(5.32±0.56)and(18.45±3.08)pmol/L before treatment;TSH of(3.12±0.56)μIU/ml was higher than that of(0.31±0.03)μIU/ml before treatment;the nodule volume of(0.32±0.03)cm3 was lower than that of(1.56±0.21)cm3 before treatment;the differences were statistically significant(P<0.05).After treatment,the mental health score was(92.41±3.43)points,the cognitive function score was(90.27±4.37)points,the social function score was(91.23±3.62)points,the emotional function score was(91.49±4.01)points,the role function score was(92.33±3.61)points and the physiological function score was(95.23±3.01)points,which were higher than those of(78.31±5.21),(75.43±5.24),(78.13±5.71),(78.13±5.71),(73.09±5.19)and(72.23±5.16)points before treatment,and the differences were statistically significant(P<0.05).Conclusion Application of ultrasound-guided minimally invasive thermal ablation in the treatment of thyroid nodules can shorten the operative time and length of hospital stay,reduce the amount of blood loss and complication rate,and improve the quality of life of patients.
