绝经后女性维生素D和维生素B_(12)水平与骨质疏松症发生风险的相关性

摘要 目的探讨绝经后女性维生素D和维生素B_(12)水平与骨质疏松症发生风险的相关性。方法选取2018年6月至2021年6月于本院治疗的128例绝经后女性骨质疏松患者作为观察组,另选取同期于本院行健康体检的128名绝经后女性作为对照组。收集两组临床资料,测定25-羟基维生素D_(3)[25-(OH)D_(3)]、维生素B_(12)水平。比较两组25-(OH)D_(3)、维生素B_(12)水平和不同部位骨密度,比较不同年龄患者25-(OH)D_(3)、维生素B_(12)及骨密度。结果观察组25-(OH)D_(3)、维生素B_(12)水平均高于对照组,差异有统计学意义(P<0.05)。两组L_(1~4)、大粗隆、大转子区、左侧股骨上端骨密度比较差异无统计学意义;观察组左侧股骨颈、Wards三角骨密度低于对照组,差异有统计学意义(P<0.05)。<60岁患者25-(OH)D_(3)、维生素B_(12)水平及左侧股骨颈、Wards三角、L_(1~4)、大粗隆、大转子区、左侧股骨上端骨密度均高于≥60岁患者,差异有统计学意义(P<0.05)。结论绝经后女性25-(OH)D_(3)及维生素B_(12)水平下降,而25-(OH)D_(3)、维生素B12水平与骨骼不同部位的骨密度密切相关,因此,应积极补充维生素等物质,降低骨质疏松症发生风险。 Objective To explore the association of vitamin D and vitamin B12 levels with the risk of postmenopausal women with osteoporosis.Methods 128 postmenopausal women with osteoporosis treated in our hospital from June 2018 to June 2021 were selected as the observation group,and another 128 postmenopausal women who underwent physical examination in our hospital during the same period were selected as the control group.The clinical data of the two groups were collected and the levels of 25-hydroxyvitamin D_(3)(25-[OH]D_(3))and vitamin B_(12)were determined,25-(OH)D_(3),vitamin B_(12)and bone mineral density in different parts were compared between the two groups,and 25-(OH)D_(3),vitamin B12 and bone mineral density in paitents at different ages were compared.Results The levels of 25-(OH)D_(3)and vitamin B_(12)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in bone mineral density of L_(1-4),greater trochanter,greater trochanter and left femur between the two groups;the bone mineral density of the left femoral neck andWards triangle in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of 25-(OH)D_(3)and vitamin B_(12)and bone mineral density of left femoral neck,Wards triangle,L_(1-4),greater trochanter,greater trochanter area and left upper femur in patients<60 years old were higher than those in patients≥60 years old,and the differences were statistically significant(P<0.05).Conclusion The levels of 25-(OH)D_(3)and vitamin B_(12)are decreased in postmenopausal women,and the levels of 25-(OH)D_(3)and vitamin B_(12)are closely related to bone mineral density in different parts of bone,therefore,vitamins and other substances should be actively supplemented to reduce the risk of osteoporosis.
