下肢动脉彩色多普勒超声检查诊断糖尿病下肢动脉病变的应用价值分析

Analysis of the application value of lower extremity arterial color Doppler ultrasonography in the diagnosis of diabetic lower extremity arterial disease

目的探讨下肢动脉彩色多普勒超声检查诊断糖尿病下肢动脉病变的应用价值。方法选取2019年6月至2020年6月本院收治的90例糖尿病患者作为研究组,另选取本院同期90名健康体检者作为参照组。两组均应用彩色多普勒超声检查下肢动脉,比较两组下肢动脉血管病变发生率、下肢动脉血管内径及血流量,以血管造影检查为金标准,分析彩色多普勒超声检查的诊断准确价值。结果研究组下肢动脉血管硬化斑块、血管狭窄与血管闭塞发生率均高于参照组,差异有统计学意义(P<0.05)。研究组股动脉、腘动脉、胫动脉与足背动脉内径均短于参照组,差异有统计学意义(P<0.05)。研究组股动脉、腘动脉、胫动脉与足背动脉血流量均明显慢于参照组,差异有统计学意义(P<0.05)。血管造影检查糖尿病下肢动脉病变78例,彩色多普勒超声检查糖尿病下肢动脉病变80例,彩色多普勒超声检查诊断准确率为93.33%,特异度为66.67%,灵敏度为97.44%。结论应用彩色多普勒超声检查诊断糖尿病患者下肢动脉病变,不但能进一步了解其病变程度,还可通过动态观察下肢动脉内径与血流量,提升诊断准确性。

Objective To explore the application value of color Doppler ultrasonography in the diagnosis of diabetic lower extremity arterial disease.Methods 90 patients with diabetes admitted to our hospital from June 2019 to June 2020 were selected as the study group,and another and 90 healthy subjects in our hospital during the same period were selected as the reference group.The lower extremity arteries were examined by color Doppler ultrasound in the two groups,the incidence of lower extremity arterial disease,the inner diameter and blood flow of lower extremity arteries were compared between the two groups,the diagnostic accuracy of color Doppler ultrasonography was analyzed with angiography as the gold standard.Results The incidence of atherosclerotic plaque,vascular stenosis and vascular occlusion in the study group were higher than those in the reference group,and the differences were statistically significant(P<0.05).The inner diameters of femoral artery,popliteal artery,tibial artery and dorsalis pedis artery in the study group were shorter than those in the reference group,and the differences were statistically significant(P<0.05).The blood flow of femoral artery,popliteal artery,tibial artery and dorsalis pedis artery in the study group were significantly slower than those in the reference group,the differences were statistically significant(P<0.05).78 cases of diabetic lower extremity arterial disease were examined by angiography,and 80 cases of diabetic lower extremity arterial disease were examined by color Doppler ultrasound,the diagnostic accuracy of color Doppler ultrasound was 93.33%,the specificity was 66.67%,and the sensitivity was 97.44%.Conclusion The application of color Doppler ultrasonography in the diagnosis of diabetic lower extremity arterial disease can not only further understand the degree of disease,but also improve the accuracy of diagnosis by dynamically observing the inner diameter and blood flow of lower extremity arteries.
