微创手术前行亮丙瑞林干预对子宫肌瘤患者病情改善价值研究

Study on the Value of Leuprolide Intervention before Minimally Invasive Surgery on the Improvement of Patients with Uterine Fibroids

目的:观察在子宫肌瘤患者行剔除术前应用亮丙瑞林作预处理的效果,为临床提供依据。方法:回顾性分析2021年1月—2022年1月焦作市妇幼保健院收治的127例子宫肌瘤患者的一般资料,基于不同的治疗方法将其分为治疗组(n=65)和常规组(n=62),治疗组患者在手术前经皮下肌肉注射亮丙瑞林,两组患者均行腹腔镜下子宫肌瘤剔除术。比较两组患者术中指标、术后指标、激素情况、预后情况。结果:治疗组患者与常规组相比,术中出血、盆腔引流量、术后下床及住院时间更低,差异有统计学意义(t=10.378、5.385、25.057、46.362,P<0.05);手术后,治疗组患者卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E_(2))、孕酮(P)改善情况更佳,更接近正常控制标准,差异有统计学意义(t=14.447、13.719、3.731、5.197,P<0.05);手术后,治疗组患者的并发症发生风险更低,差异有统计学意义(χ^(2)=0.754,P<0.05);治疗后随访期间,治疗组患者的月经周期、经量及子宫体积改善情况更佳,差异有统计学意义(t=64.830、4.564、8.801,P<0.05)。结论:采用亮丙瑞林作子宫肌瘤剔除术前预处理的效果更好,对促进患者术后恢复、改善患者预后均有积极意义。

Objective:To observe the effect of pre-treatment with leuprolide in patients with uterine fibroids before excisional surgery to provide a clinical basis.Methods:The general data of 127 patients with uterine fibroids admitted to the hospital from January 2021 to January 2022 were retrospectively analyzed and divided into treatment group(n=65)and conventional group(n=62)based on different treatment methods.In the treatment group,leuprolide was injected subcutaneously intramuscularly before surgery,and patients in both groups underwent laparoscopic myomectomy.The intraoperative indexes,postoperative indexes,hormonal status,and prognosis of the patients in the two groups were compared.Results:Intraoperative bleeding,pelvic drainage,postoperative bed and hospital stay were lower in the treatment group compared with the conventional group,with statistically significant differences(t=10.378,5.385,25.057,46.362,P<0.05).After surgery,folliculogenic hormone(FSH),luteinizing hormone(LH),estradiol(E_(2)),and progesterone(P)improved better and were closer to normal control criteria in the treatment group,with statistically significant differences(t=14.447,13.719,3.731,5.197,P<0.05).After surgery,the risk of complications was lower in the treatment group,with a statistically significant difference(χ^(2)=0.754,P<0.05).During post-treatment follow-up,the treatment group had better improvement in menstrual cycle,menstrual volume and uterine volume,with statistically significant differences(t=64.830,4.564,8.801,P<0.05).Conclusion:The use of leuprolide as preoperative pretreatment for myomectomy is more effective and has positive implications for promoting postoperative recovery and improving patient prognosis.
