基质金属蛋白酶、金属蛋白酶组织抑制剂联合癌胚抗原、糖类抗原19-9检测在早期胃癌诊断和评估中的应用

Application of matrix metalloproteinases and tissue inhibitors of metalloproteinases combined with carcinoembryonic antigen and carbohydrate antigen 19-9 detection in the diagnosis and evaluation of early gastric cancer

目的分析基质金属蛋白酶(MMP-9)、金属蛋白酶组织抑制剂(TIMP-2)联合癌胚抗原(CEA)、糖类抗原19-9(CA19-9)检测在早期胃癌诊断和评估中的应用价值。方法选取2021年8月至2022年8月福清市医院收治89名早期胃癌患者和98名健康对照者作为研究对象。采用ELISA法分析MMP-9、TIMP-2水平,使用自动免疫分析仪测量CEA和CA19-9水平。比较两组间MMP-9、TIMP-2、CEA和CA19-9水平的差异;绘制受试者工作特征(ROC)曲线分析MMP-9、TIMP-2、CEA和CA19-9单独和联合检测对早期胃癌诊断的价值。结果早期胃癌组患者的血清MMP-9、TIMP-2、CEA、CA19-9表达水平均高于健康对照组,差异有统计学意义(P<0.01)。血清CA19-9、MMP-9和TIMP-2联合检测的诊断价值高于单一检测。结论血清MMP-9、TIMP-2联合CA19-9检测具有较高的灵敏度和特异性,对于早期胃癌的筛查与诊断具有较高的临床应用价值。

Objective To analyze the application value of matrix metalloproteinase 9(MMP-9)and tissue inhibitor of metalloproteinase 2(TIMP-2)combined with carcinoembryonic antigen(CEA)and carbohydrate antigen 19-9(CA19-9)detection in the diagnosis and evaluation of early gastric cancer.Methods A total of 89 patients with early gastric cancer and 98 healthy controls were selected from Fuqing Hospital from August 2021 to August 2022.The levels of MMP-9 and TIMP-2 were analyzed by ELISA,and CEA and CA19-9 were measured by automatic immunoanalyzer.The levels of MMP-9,TIMP-2,CEA and CA19-9 were compared between the two groups.Receiver operating characteristic(ROC)curve was plotted to analyze the value of MMP-9,TIMP-2,CEA and CA19-9 alone and in combination in the diagnosis of early gastric cancer.Results The expression levels of MMP-9,TIMP-2,CEA and CA19-9 in patients with early gastric cancer group were higher than those in healthy control group,and the differences were statistically significant(P<0.01).The diagnostic value of combined detection of serum CA19-9,MMP-9 and TIMP-2 was higher than that of single detection.Conclusion The combination of serum MMP-9,TIMP-2 and CA19-9 detection has high sensitivity and specificity,and has a high clinical application value for the screening and diagnosis of early gastric cancer.
