延续护理在肩关节镜下肩袖损伤修复术后患者中的应用效果

Effects of extended nursing on muscle strength recovery and daily living ability of patients after arthroscopic rotator cuff injury repair operation of the shoulder

目的 探讨延续护理模式对肩关节镜下肩袖损伤修复术后患者的影响。方法 选取2020年1月-2022年12月常州市中医医院骨科收治的80例肩袖损伤患者作为研究对象,按照组间基本特征具有可比性的原则分为对照组和观察组,每组40例。对照组实施围术期常规护理,观察组在此基础上,实施延续护理干预。比较两组患者干预6个月后的肌力恢复、肩关节功能评分、肩关节活动度、日常生活能力评分。结果 护理干预前,两组患者患侧肩关节前屈、后伸、内旋、外旋最大角度及Constant-Murley肩关节功能评分、日常生活活动能力量表(ADL)评分的比较,差异无统计学意义(P>0.05);护理干预6个月后,观察组患者Constant-Murley肩关节功能评分、ADL评分高于对照组,差异有统计学意义(P<0.05);观察组患侧肩关节前屈、后伸、内旋、外旋最大角度大于对照组,差异有统计学意义(P<0.05);观察组肌力恢复优于对照组,差异有统计学意义(P<0.05)。结论 延续护理应用于肩袖损伤行肩关节镜下修复术治疗患者,可有效促进其肌力恢复,改善肩关节活动度,有利于促进肩关节功能恢复,从而提高日常生活能力。

Objective To explore the effects of extended care model on muscle strength recovery and daily life ability of patients after arthroscopic rotator cuff injury repair.Methods 80 rotator cuff injury patients admitted to the Department of Orthopaedics of Changzhou Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected as the study subjects,they were divided into a control group and an observation group according to the principle basic on the characteristics between the groups were comparable,with 40 cases in each group.The control group implemented perioperative routine care,the observation group implemented extended nursing intervention on the basis of control group.The muscle strength recovery,shoulder joint function score,shoulder joint mobility,and daily life ability score of the two groups were compared after 6 months of intervention.Results Before nursing intervention,there was no statistically significant difference in the comparison of the maximum angles of forward flexion,backward extension,internal rotation and external rotation of the aff ected shoulder joints and the Constant-Murley shoulder joint function scores and the scores of Activity of Daily Living ability scale(ADL)of the two groups of patients(P>0.05).After 6 months of nursing intervention,the Constant-Murley shoulder joint function scores of the observation group patients and ADL scores were higher than those of the control group,and the difference was statistically signifi cant(P<0.05),the maximum angles of anterior flexion,posterior extension,internal rotation and external rotation of the aff ected shoulder joints of the observation group were higher than the control group,the diff erence was statistically significant(P<0.05);and the recovery of muscle strength of the observation group was better than that of the control group,and the diff erence was statistically significant(P<0.05).Conclusion Extended care applied to patients treated with shoulder arthroscopic repair for rotator cuff injury could eff ectively promote the recovery of muscle strength,improve the mobility of the shoulder joint and conducive to promoting the recovery of shoulder joint function,thus improving the ability of daily life.
