骨运输术中使用导针定位辅助单边外固定架螺钉置入效果分析

Analysis of the effect of using guide pin positioning to assist unilateral external fixator screw placement in bone transport

摘要 目的:探讨骨运输术中使用导针定位辅助单边外固定架螺钉置入的效果。方法:采用假骨模型制成胫骨中段缺损10 cm的样本30例。将30例胫骨缺损模型,随机分为试验组和对照组,各15例。对照组采用传统方法,试验组采用导针定位辅助单边重建外固定架螺钉置入法,比较两组临床疗效、准确螺钉评分、X线透视次数、操作时间等。结果:试验组固定有效率(93.33%)高于对照组(53.33%),准确螺钉评分(92.7±7.3)分高于对照组(82.1±9.0)分,X线透视次数(1.2±0.3)次少于对照组(3.0±1.5)次,操作时间(0.8±0.4)h短于对照组(3.4±1.6)h,差异均有统计学意义(P<0.05)。结论:骨运输术中使用导针定位辅助单边重建外固定架螺钉置入法具有操作简单、创伤小、准确性高、操作时间短、疗效显著等特点。 Objective:To investigate the effect of using guide pin positioning to assist unilateral external fixator screw placement in bone transport.Methods:A total of 30 cases with 10 cm of tibial interrupt defect were made by using pseudobone model.Thirty cases of tibial defect model were randomly divided into experimental group and control group with 15 cases each.The traditional method was used in the control group,and the new method of screw placement in the unilateral reconstruction external fixator was used in the experimental group during bone transport.The clinical effect,X-ray fluoroscopy frequency and operation time were compared between the two groups.Results:The fixed effective rate(93.33%)and correct screw score(92.7±7.3)of experimental group were higher than that of control group(53.33%,82.1±9.0);The times of X-ray fluoroscopy(1.2±0.3)were less than that of control group(3.0±1.5),and the operation time[(0.8±0.4)h]was shorter than that of control group[(3.4±1.6)h],with statistical significance(P<0.05).Conclusion:The screw placement of unilateral reconstruction external fixator in bone transport has the characteristics of simpler operation,less trauma,higher accuracy,shorter operation time and significant curative effect.
