穴位刺激配合乳房按摩改善剖宫产产妇产后缺乳的效果

Effect of acupoint stimulation combined with breast massage on improving postpartum breast deficiency of caesarean section parturients

目的观察穴位刺激配合乳房按摩改善剖宫产产妇产后缺乳的临床效果。方法采用便利抽样法,在2019年1月至2020年12月在山东省淄博市第一医院产科接受剖宫产分娩的产妇中,纳入150例产后缺乳产妇进行研究,按照随机数字表法将其分为观察组和对照组,各75例。对照组采用常规产后护理,观察组在对照组基础上采用穴位刺激配合乳房按摩。比较两组干预前后乳汁分泌量评分、中医症候积分及血清催乳素水平。比较两组出院后新生儿母乳喂养率及纽卡斯尔护理满意度量表(NSNS)评分。结果组内比较:两组各时间点乳汁分泌量评分两两比较,差异有统计学意义(P<0.05);组间比较:干预后1、2、3 d,观察组产妇乳汁分泌量评分均高于对照组(P<0.05)。干预后,两组乳房充盈程度、乳汁淤积程度及乳汁黏稠程度评分较干预前降低,且观察组低于对照组(P<0.05)。出院后,观察组产妇新生儿母乳喂养率高于对照组(P<0.05)。干预后,观察组NSNS评分高于对照组(P<0.05)。结论穴位刺激配合乳房按摩能够有效改善剖宫产产后产妇缺乳症状,提高其乳房充盈度及黏稠程度、降低乳汁淤积程度,提高新生儿的母乳喂养率及产妇对护理的满意度。

Objective To observe the clinical effect of acupoint stimulation combined with breast massage on improving postpartum breast deficiency in caesarean section parturients.Methods From January 2019 to December 2020,150 parturients who received caesarean section delivery in the Department of Obstetrics,Zibo First Hospital of were included in the study.According to the random number table method,they were divided into the observation group and the control group,with 75 cases in each group.The control group adopted routine postpartum care,while the observation group adopted acupoint stimulation combined with breast massage on the basis of the control group.The scores of milk secretion,traditional Chinese medicine score,and serum prolactin level were compared between the two groups before and after nursing.Neonatal breastfeeding rates and Newcastle nursing satisfaction scale(NSNS)scores were compared between the two groups after discharge.Results Intra-group comparison:there was statistical significance in the score of milk secretion between the two groups at each time point(P<0.05).Inter-group comparison:one,two and three days after intervention,the scores of maternal milk secretion in observation group were higher than those in control group(P<0.05).The scores of breast filling degree,milk siltation degree,and milk viscosity degree in two groups were lower than before intervention,and those in the observation group were lower than control group(P<0.05).After discharge,the neonatal breastfeeding rate in the observation group was higher than that in the control group(P<0.05).After intervention,the maternal NSNS score in observation group was higher than that in control group(P<0.05).Conclusion Acupoint stimulation combined with breast massage can effectively improve the symptoms of breast deficiency of parturients after cesarean section,improve their breast filling and viscosity,reduce the degree of milk stasis,and improve the rate of breast feeding of newborns.
