右美托咪定复合瑞芬太尼对颈部瘢痕挛缩畸形患者应激状态及呼吸状态的影响

摘要 目的探讨右美托咪定复合瑞芬太尼对颈部瘢痕挛缩畸形患者应激状态及呼吸状态的影响。方法选取2019年5月至2022年5月收治的80例颈部瘢痕挛缩畸形患者为研究对象,均行手术治疗,根据麻醉方式不同将其分为对照组和观察组,各40例。对照组采用丙泊酚复合瑞芬太尼维持麻醉,观察组采用右美托咪定复合瑞芬太尼维持麻醉。比较两组的麻醉效果。结果手术切皮即刻(T_(2))至术毕即刻(T_(5)),观察组的皮质醇(Cor)、肾上腺素(E)水平低于对照组,呼吸频率(RR)、血氧饱和度(SpO_(2))高于对照组(P<0.05)。观察组的苏醒时间短于对照组,术后2、6 h视觉模拟评分法(VAS)评分低于对照组(P<0.05)。结论临床对颈部瘢痕挛缩畸形患者麻醉时采用右美托咪定复合瑞芬太尼有助于减轻其应激状态,维持呼吸状态稳定,也能缩短苏醒时间,提高术后镇痛镇静效果。 Objective To investigate the effects of dexmedetomidine combined with remifentanil on stress state and respiratory state in patients with cervical scar contracture deformity.Methods A total of 80 patients with cervical scar contracture deformity admitted from May 2019 to May 2022 were selected as the research objects.All patients underwent surgical treatment and were divided into control group and observation group according to different anesthesia methods,with 40 cases in each group.The control group was adopted with propofol combined with remifentanil to maintain anesthesia,and the observation group was adopted with dexmedetomidine combined with remifentanil to maintain anesthesia.The anesthetic effects of the two groups were compared.Results From immediately after skin incision(T_(2))to immediately after operation(T_(5)),the levels of cortisol(Cor)and epinephrine(E)in the observation group were lower than those in the control group,and the respiratory rate(RR)and oxygen saturation(SpO_(2))were higher than those in the control group(P<0.05).The recovery time of the observation group was shorter than that of the control group,and the Visual Analog Scale(VAS)score at 2 and 6 h after operation was lower than that of the control group(P<0.05).Conclusion The clinical use of dexmedetomidine combined with remifentanil during anesthesia in patients with cervical scar contracture deformity can help reduce their stress state,maintain stable respiratory state,shorten the recovery time,and improve the effect of postoperative analgesia and sedation.
