摘要
目的分析血尿酸(UA)对肥胖人群动脉硬化预测价值。方法选取体检者12259例,根据体质指数(BMI)分为肥胖组1875例和非肥胖组10384例,根据踝臂脉搏波传导速度(baPWV)分为动脉硬化组和无动脉硬化组。记录所有受检者一般资料以及血压、血尿酸、血糖、血脂和baPWV。采用多元线性回归分析肥胖人群baPWV的影响因素。采用Spearman相关性分析血尿酸与影响因素的相关性。ROC曲线分析血尿酸、BMI对肥胖人群动脉硬化的诊断价值。结果12259例体检者中肥胖者1875例(15.29%),肥胖人群动脉硬化检出905例(7.38%)。动脉硬化组血UA水平明显高于无动脉硬化组(P<0.05);肥胖组与非肥胖组、动脉硬化组与无动脉硬化组的BMI、收缩压(SBP)、舒张压(DBP)、空腹血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDLC)、低密度脂蛋白胆固醇(LDLC)差异均存在显著性(P<0.05)。Spearman相关分析显示血UA与baPWV、SBP、DBP、BMI、FBG、TG呈正相关,而与HDLC呈负相关;SBP、DBP、BMI、TC、LDLC、FBG、UA均是动脉硬化的影响因素(P<0.05)。ROC结果显示,血UA>350.05μmol/L联合BMI>24.45 kg/m2对肥胖人群发生动脉硬化具有较高预测价值。结论肥胖人群伴血UA高于350.05μmol/L者更易发生动脉硬化。
Aim To analyze the predictive value of blood uric acid on atherosclerosis in obese individuals.Methods 12259 physical examination subjects were selected and divided into the obese group of 1875 cases and the nonobese group of 10384 cases based on body mass index(BMI).According to ankle brachial pulse wave conduction velocity(baPWV),they were divided into the arteriosclerosis group and the non-arteriosclerosis group.General data of all individuals and blood pressure,uric acid,blood sugar,blood lipids,and baPWV were recorded.Multiple linear regression was used to analyze the influencing factors of baPWV in obese individuals.The correlation between blood uric acid and influencing factors was analyzed by Spearman correlation.The diagnostic value of blood uric acid combining with BMI in obese individuals with arteriosclerosis was analyzed by ROC curve.Results Among the 12259 physical examination individuals,1875 cases(15.29%)were obese,and 905 cases(7.38%)were found to have atherosclerosis in the obese population.Blood UA levels in the arteriosclerosis group is significantly higher that in the non-arteriosclerosis group(P<0.05).There were significant differences in BMI,systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FBG),triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDLC),and low-density lipoprotein cholesterol(LDLC)between the obese group and the non-obese group,as well as between the arteriosclerosis group and the non-arteriosclerosis group(P<0.05).Spearman correlation analysis showed that blood UA was positively correlated with baPWV,SBP,DBP,BMI,FBG,TG,and negatively correlated with HDLC.SBP,DBP,BMI,TC,LDLC,FBG,UA are all influencing factors of arteriosclerosis(P<0.05).ROC analysis showed that blood UA>350.05μmol/L and BMI>24.45 kg/m2 has a high predictive value for the occurrence of arteriosclerosis in obese individuals.Conclusion Obese individuals with blood UA levels higher than 350.05μmol/L is more prone to arteriosclerosis.
