3D优化仿真手法复位辅助经皮椎体成形术治疗椎体压缩骨折的效果观察

摘要 目的:探究3D优化仿真手法复位辅助经皮椎体成形术(PVP)治疗椎体压缩骨折的效果。方法:选取2016年6月—2023年1月东莞市石排医院收治的椎体压缩骨折患者58例作为研究对象,采用单双号法随机分为手法辅助组和常规手术组,各29例。常规手术组采用PVP治疗,手法辅助组在常规手术组基础上应用3D优化仿真手法复位辅助。比较两组手术前后椎体前缘高度、后凸Cobb角度、疼痛数字评价量表(NRS)评分、腰椎功能障碍指数问卷表(ODI)评分。结果:术后2个月,两组患者椎体前缘高度高于术前,后凸Cobb角度小于术前,差异有统计学意义(P<0.05),其中手法辅助组椎体前缘高度高于常规手术组,后凸Cobb角度小于常规手术组,差异有统计学意义(P<0.001);术后2个月,两组NRS、ODI评分低于术前,手法辅助组低于常规手术组,差异有统计学意义(P<0.05)。结论:3D优化仿真手法复位辅助PVP可恢复患者椎体高度,缓解患者术后疼痛,改善脊柱生理曲度及腰椎功能,建议临床应用并予以推广。 Objective:To investigate the effect of 3D optimization simulation manual reduction assisted percutaneous vertebroplasty(PVP)in the treatment of vertebral compression fractures.Methods:A total of 58 patients with vertebral compression fracture treated in Shipai Hospital of Dongguan City from June 2016 to January 2023 were selected as the study subjects.They were randomly divided into manipulation assisted group and conventional surgery group by odd-even number method,with 29 cases in each group.The conventional surgery group was treated with PVP,and the manipulation assisted group was assisted with 3D optimization simulation manual reduction on the basis of the conventional surgery group.The anterior vertebral margin height,kyphotic Cobb angle,pain Numerical Rating Scale(NRS)score and lumbar dysfunction Oswestry Disability Index(ODI)score were compared between the two groups before and after surgery.Results:At 2 months after surgery,the anterior vertebral margin height in two groups was higher than that before surgery,and the kyphotic Cobb angle in two groups was lower than that before surgery,and the difference was statistically significant(P<0.05);the anterior vertebral margin height in the manipulation assisted group was higher than that in the conventional surgery group,and the kyphotic Cobb angle in the manipulation assisted group was lower than that in the conventional surgery group,with a significant difference(P<0.001);at 2 months after surgery,the NRS and ODI scores in the two groups were lower than those before surgery,and the scores in the manipulation assisted group were lower than those in the conventional surgery group,and the difference was statistically significant(P<0.05).Conclusion:3D optimization simulation manual reduction assisted PVP can restore the vertebral height of patients,relieve postoperative pain,improve the physiological curvature of the spine and lumbar function,and it is recommended to be used and popularized in clinical practice.
