计算机辅助设计/计算机辅助制作椅旁数字化高嵌体修复治疗大面积牙体缺损患者的效果

摘要 目的:观察计算机辅助设计/计算机辅助制作(CAD/CAM)椅旁数字化高嵌体修复治疗大面积牙体缺损患者的效果。方法:选取2020年1月至2022年12月该院收治的88例大面积牙体缺损患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各44例。对照组行常规全瓷冠修复治疗,观察组行CAD/CAM椅旁数字化高嵌体修复治疗,两组均随访6个月。比较两组修复优良率,修复前后牙周指标[牙龈指数(GI)、菌斑指数(PLI)、探诊深度(PD)]水平、龈沟液炎性因子[白细胞介素-6(IL-6)、碱性磷酸酶(ALP)、基质金属蛋白酶-8(MMP-8)]水平,以及满意度。结果:随访6个月,观察组修复优良率为97.73%(43/44),高于对照组的81.82%(36/44),差异有统计学意义(P<0.05);修复后3个月,观察组GI、PLI和PD水平均低于对照组,差异有统计学意义(P<0.05);修复后3个月,观察组龈沟液IL-6、ALP、MMP-8水平均低于对照组,差异有统计学意义(P<0.05);观察组满意度为95.45%(42/44),高于对照组的77.27%(34/44),差异有统计学意义(P<0.05)。结论:CAD/CAM椅旁数字化高嵌体修复治疗大面积牙体缺损患者可提高修复优良率和满意度,降低牙周指标和龈沟液炎性因子水平,效果优于常规全瓷冠修复治疗。 Objective:To observe effects of computer-aided design/computer-aided manufacturing(CAD/CAM)digital chair-side onlay restoration in treatment of patients with large area tooth defects.Methods:A prospective study was conducted on 88 patients with large area tooth defects admitted to this hospital from January 2020 to December 2022.They were divided into control group and observation group according to the random number table method,44 cases in each.The control group was treated with conventional all-ceramic crown restoration,while the observation group was treated with CAD/CAM digital chair-side onlay restoration.Both groups were followed up for 6 months.The excellent and good rate of restoration,the levels of periodontal indexes[gingival index(GI),plaque index(PLI),probing depth(PD)],the inflammatory factor levels[interleukin-6(IL-6),alkaline phosphatase(ALP),matrix metalloproteinase-8(MMP-8)]in gingival crevicularfluid before and after the restoration,and the satisfaction were compared between the two groups.Results:After 6 months of follow-up,the excellent and good rate of restoration in the observation group was 97.73%(43/44),which was higher than 81.82%(36/44)in the control group,and the difference was statistically significant(P<0.05).3 months after the restoration,the levels of GI,PLI and PD in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).3 months after the restoration,the levels of IL-6,ALP and MMP-8 in gingival crevicularfluid of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Further,the satisfaction rate of the observation group was 95.45%(42/44),which was higher than 77.27%(34/44)of the control group,and the difference was statistically significant(P<0.05).Conclusions:CAD/CAM chairside digital onlay restoration in the treatment of the patients with large area tooth defects can improve the excellent and good rate of restoration and the satisfaction and reduce the levels of periodontal indexes and gingival crevicularfluid inflammatory factors.Moreover,it is superior to conventional all-ceramic crown restoration.
