肌功能矫治器联合直丝弓矫治技术治疗上颌前突患者的效果

Effects of myofunctional appliance combined with straight arch wire technique in treatment of patients with maxillary protrusion

目的:观察肌功能矫治器联合直丝弓矫治技术治疗上颌前突患者的效果。方法:选取2020年1月至2021年12月该院收治的76例上颌前突患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各38例。对照组应用直丝弓矫治技术治疗,观察组在对照组基础上联合肌功能矫治器治疗,比较两组上颌骨指标[上颌突度(SNA)、下颌突度(SNB)、上颌中切牙对前颅底平面的倾斜角(U1-SN)、上颌中切牙端至眼耳平面的垂直距离(U1-FN)、上颌第一磨牙近中牙尖腭平面距离(U6-PP)]水平、咀嚼效率和治疗满意度。结果:治疗后,两组SNA、U1-SN水平低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组SNB、U1-FN、U6-PP水平比较,差异无统计学意义(P>0.05);治疗后,两组咀嚼效率均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗满意度为97.37%,明显高于对照组的78.95%,差异有统计学意义(P<0.05)。结论:肌功能矫治器联合直丝弓矫治技术治疗上颌前突患者可提高咀嚼效率和治疗满意度,降低SNA和U1-SN水平,效果优于单纯直丝弓矫治技术治疗。

Objective:To observe effects of myofunctional appliance combined with straight arch wire technique in treatment of patients with maxillary protrusion.Methods:A prospective study was conducted on 76 patients with maxillary protrusion admitted to this hospital from January 2020 to December 2021.They were divided into observation group and control group according to the random number table method,38 cases in each group.The control group was treated with straight arch wire technique,while the observation group was treated with myofunctional appliance on the basis of that of the control group.The maxillary indexes[maxillary protrusion(SNA),mandibular protrusion(SNB),inclination angle of maxillary central incisor to anterior skull base plane(U1-SN),vertical distance from maxillary central incisor end to eye and ear plane(U1-FN),maxillary first molar apical plane distance(U6-PP)]level,the masticatory efficiency,and the treatment satisfaction were compared between the two groups.Results:After the treatment,the levels of SNA and U1-SN in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,there was no statistically significant differences in the levels of SNB,U1-FN,and U6-PP between the two groups(P>0.05).After the treatment,the masticatory efficiency of the two groups were higher than those before the treatment,those of the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the treatment satisfaction of the observation group was 97.37%,which was significantly higher than 78.95%of the control group,and the difference was statistically significant(P<0.05).Conclusions:Myofunctional appliance combined with straight arch wire technique in the treatment of the patients with maxillary protrusion can improve the masticatory efficiencies and the treatment satisfaction,and reduce the SNA and U1-SN levels.Moreover,it is superior to single straight arch wire technique treatment.
