循经针刺治疗腰椎间盘突出症的效果及对腰椎功能和血清炎症因子水平的影响

Effect of Acupuncture along Meridian on Lumbar Disc Herniation and Its Influence on Lumbar Function and Serum Inflammatory Factors

目的:观察循经针刺治疗腰椎间盘突出症的效果及对腰椎功能和血清炎症因子水平的影响。方法:研究对象为2020年1月—2021年12月江西省中西医结合医院收治的80例腰椎间盘突出症患者,将其随机分为对照组和观察组,每组40例。对照组采取常规西药治疗,观察组采用西药联合循经针刺治疗。比较两组症状积分、腰椎功能[日本骨科协会评估治疗(JOA)评分]、血清炎症因子[C反应蛋白(CRP)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)]水平、下肢功能障碍情况[直腿抬高角度、Oswestry功能障碍指数(ODI)]及疗效。结果:治疗后观察组各项症状积分及总分均低于对照组(P<0.05);治疗后观察组JOA各维度评分及总分均高于对照组(P<0.05);治疗后两组CRP、IL-6及TNF-α水平均降低,且观察组均低于对照组(P<0.05);治疗后两组ODI评分均下降,且观察组较对照组更低(P<0.05),两组直腿抬高角度均大于治疗前,且观察组较对照组更大(P<0.05);观察组总有效率较对照组更高(P<0.05)。结论:循经针刺治疗腰椎间盘突出症的效果明显,能有效改善患者的腰椎功能及血清炎症因子水平。

Objective:To observe the effect of acupuncture along meridian on lumbar disc herniation and its influence on lumbar function and serum inflammatory factors.Method:A total of 80 patients with lumbar disc herniation admitted to Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to December 2021 were selected as study objects and randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with western medicine combined with acupuncture along meridian.The symptoms integrations,lumbar spine function[Japanese orthopaedic association(JOA)scores],serum inflammatory factors[C reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]levels,lower limb dysfunction condition[straight leg raising angle and Oswestry disability index(ODI)]and efficacy were compared between the two groups.Result:After treatment,the various symptoms integrations and total integrations in the observation group were lower than those in the control group(P<0.05).After treatment,the each dimension scores of JOA and total scores of the observation group were higher than those of the control group(P<0.05).After treatment,the levels of CRP,IL-6 and TNF-αwere decreased in both groups,and those in the observation group were lower than those in the control group(P<0.05).After treatment,the ODI scores were decreased in both groups,and that in the observation group was lower than that in the control group(P<0.05),the straight leg raising angle in both groups were greater than those before treatment,and that in the observation group was larger than that in the control group(P<0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion:The effect of acupuncture along meridian in the treatment of lumbar disc herniation is obvious,which can effectively improve the function of lumbar spine and the levels of serum inflammatory factors.
