慢性支气管炎缓解期伏九穴位敷贴临床疗效观察

Observation on the clinical curative effect of Fujiutie acupoint application in the remission period of chronic bronchitis

目的:比较夏季三伏贴、冬季三九贴、冬夏伏九贴治疗慢性支气管炎缓解期的临床疗效及中医证候疗效,探讨慢性支气管炎防治最佳时机,优选最佳疗程方案,指导临床。方法:收集2021年7月—2021年12月,就诊于上海市虹口区凉城新村街道社区卫生服务中心中医科门诊的慢性支气管炎缓解期患者90例,采用随机对照、重复测量法分为夏季三伏组、冬季三九组、冬夏伏九组,每组30例,除去脱落病例,最终纳入夏季三伏组29例、冬季三九组28例、冬夏伏九组29例进行统计分析。治疗1年,观察评价3种方案临床疗效性指标及中医证候疗效。结果:治疗1年,夏季三伏组、冬季三九组、冬夏伏九组患者中医证候有效率分别为51.7%、42.9%、79.3%,差异有统计学意义(P<0.05);3组患者咳嗽症状积分、视觉模拟评分(VAS)、每年急性发作次数均较前改善,且冬夏伏九组改善明显,差异有统计学意义(P<0.05)。结论:与夏季三伏、冬季三九两种疗法比较,冬夏伏九疗法中医证候疗效更佳,临床疗效性指标改善更明显。因此,冬夏伏九疗法可成为防治慢性支气管炎缓解期优选外治方案。

Objective:To compare the clinical efficacy and traditional Chinese medicine(TCM)syndrome efficacy of Sanfutie in summer,Sanjiutie in winter and Fujiutie in winter and summer in the treatment of chronic bronchitis in remission stage,to explore the best time to prevent and treat chronic bronchitis,and to choose the best course of treatment for guiding clinical practice.Methods:From July 2021 to December 2021,90 patients with chronic bronchitis in remission period in the outpatient department of Traditional Chinese Medicine of Liangcheng Xincun Street Community Health Service Center,Hongkou District of Shanghai were selected.The patients were divided into three groups by randomized controlled method and repeated measurement method:Sanfutie group in summer,Sanjiutie group in winter and Fujiutie group in winter and summer,with 30 cases in each group.Excluding the dropped cases,29 cases in the Sanfutie in summer,28 cases in the Sanjiutie in winter,and 29 cases in the Fujiutie in winter and summer were finally included for statistical analysis.After 1 year of treatment,the clinical efficacy indexes and TCM syndromes curative effect of the 3 schemes were observed and evaluated.Results:After 1 year of treatment,the effective rates of TCM syndromes in the Sanfutie group in summer,Sanjiutie group in winter and Fujiutie group in winter and summer were 51.7%,42.9%,and 79.3%,respectively,and the differences were statistically significant(P<0.05).The scores of cough symptoms,visual analogue scale(VAS)and the number of acute attacks per year in the 3 groups were improved compared with those before treatment,and the improvements were significant in the Fujiutie group in winter and summer with statistical significance(P<0.05).Conclusion:Compared with Sanfutie in summer and Sanjiutie in winter,Fujiutie in winter and summer has better curative effect on TCM syndromes,and the improvement of clinical efficacy index is more obvious.Therefore,Fujiutie in winter and summer can be the preferred external treatment for chronic bronchitis in the remission period.
