外周血CD161检测及GeneXpert联合检测在肺结核患者鉴别诊断中的临床价值

Clinical value of peripheral blood CD161 detection and GeneXpert combined detection in differential diagnosis of PTB patients

目的探讨外周血CD161检测联合GeneXpert检测在肺结核患者(PTB)诊断中的临床价值。方法回顾性分析本院2022年1-8月期间收治的202例疑似肺结核患者的实验室检查结果,比较外周血CD161检测、GeneXpert检测对PTB的检测灵敏度、特异度及正确指数的差异,以及二者联合应用的临床诊断效能。结果外周血CD161检测诊断PTB的灵敏度为81.93%,特异度为66.67%,正确指数0.54;GeneXpert检测PTB的灵敏度为37.95%,特异度为94.4%,正确指数0.32,二者PTB的诊断阳性率比较,差异有统计学意义(P<0.001)。而CD161检测联合GeneXpert检测在PTB中的诊断灵敏度为88.55%,特异度为72.22%,正确指数0.61。结论外周血CD161检测联合GeneXpert检测可弥补GeneXpert检测灵敏度、外周血CD161检测特异度不足的缺陷,全面性提高PTB的临床诊断效能。

Objective To investigate the clinical value of peripheral blood CD161 detection combined with GeneXpert detection in the diagnosis of pulmonary tuberculosis(PTB)patients.Methods The sensitivity,specificity and Youden′s index of peripheral blood CD161 and GeneXpert in diagnosis of PTB were compared in 202 patients with suspected pulmonary tuberculosis admitted to the Second People′s Hospital of Weifang from January to August 2022.Results The sensitivity of peripheral blood CD161 detection for the diagnosis of PTB was 81.93%,the specificity was 66.67%,and the Youden′s index was 0.54;the sensitivity of GeneXpert detection of PTB was 37.95%,the specificity was 94.4%,and the Youden′s index was 0.32.There were significant differences in the diagnostic positive rate of PTB between GeneXpert and PTB detection,and the difference was statistically significant(P<0.001).The diagnostic sensitivity of CD161 detection combined with GeneXpert detection in PTB was 88.55%,the specificity was 72.22%,and the Youden′s index was 0.61.Conclusion Peripheral blood CD161 detection combined with GeneXpert detection can make up for the deficiencies of GeneXpert detection sensitivity and peripheral blood CD161 detection specificity,and comprehensively improve the clinical diagnostic efficiency of PTB.
