冠心病合并亚临床甲减与心血管疾病标志物的相关性研究

摘要 目的探讨冠心病合并亚临床甲减与心血管疾病标志物相关性。方法纳入2019年1月至2019年12月云南省阜外心血管病医院冠心病区住院患者,行冠状动脉造影检查和甲状腺功能血清学检测,分为冠心病合并亚临床甲减组(冠心病合并亚临床甲减组,n=195),冠心病合并甲功正常组(冠心病组,n=189)。结果两组N端脑钠肽前体(NT-ProBNP)、高敏肌钙蛋白-I(Hs-TnI)相比对照组差异有统计学意义(P<0.05),两组血脂差异无统计学意义;在慢性心衰及心肌损伤患病率中,冠心病合并亚临床甲减组的患病率都高于冠心病组,差异有统计学意义(P<0.05);在冠心病基础上,促甲状腺素(TSH)为心肌损伤的独立危险因素。结论患者在冠心病基础上,对并发症的评估及诊疗过程中,应重视亚临床甲减因素,及时发现和及时干预,可能有助于心血管疾病的预防和治疗。 Objective To explore the correlation between coronary heart disease complicated with subclinical hypothyroidism and cardiovascular disease markers.Methods Inpatients with coronary heart disease in Fuwai Cardiovascular Hospital of Yunnan Province from January 2019 to December 2019 were enrolled,who underwent coronary angiography and serum detection of thyroid function,and were divided into coronary heart disease combined with subclinical hypothyroidism group(n=195)and coronary heart disease combined with normal thyroid function group(control group,n=189).Results There were statistically significant differences in NT-probNP and HS-TNI between the two groups compared with the control group(P<0.05),but there was no statistically significant difference in blood lipid between the two groups.In the prevalence of chronic heart failure and myocardial injury,the prevalence of coronary heart disease combined with subclinical hypothyroidism was higher than that of coronary heart disease group,and the difference was statistically significant(P<0.05).On the basis of coronary heart disease,TSH is an independent risk factor for myocardial injury.Conclusion On the basis of coronary heart disease,patients should pay attention to subclinical hypothyroidism factors in the evaluation and diagnosis and treatment of complications,timely detection and timely intervention,which may contribute to the prevention and treatment of cardiovascular diseases.
