128层螺旋CT联合3.0T磁共振-肝脏容积加速采集成像(多期动态增强)及弥散加权成像系列对前列腺癌分期诊断价值的前瞻性研究

目的:探究128层螺旋CT联合3.0T MR肝脏容积加速采集成像(LAVA)(多期动态增强)、弥散加权成像(DWI)系列对前列腺癌分期诊断价值。方法:选取2021年6月~2022年3月经阳春市人民医院临床实验室前列腺特异抗原、直肠指检诊断为前列腺癌的37例患者为研究对象,分别采用CT+3.0T MR LAVA(多期动态增强)、CT+3.0T MR LAVA(多期动态增强)+DWI系列检查,观察两种检查方法的A、B、C、D期患者诊断效果。结果:CT+3.0T MR LAVA诊断A、B、C、D分期结果均低于CT+3.0T MR LAVA+DWI系列(P>0.05);37例前列腺癌患者,采用CT+3.0T MR LAVA阳性28例,阴性9例,CT+3.0T MR LAVA+DWI系列阳性36例,阴性1例。CT+3.0T MR LAVA前列腺癌的阳性检出率51.35%,灵敏度75.66%均低于CT+3.0T MR LAVA+DWI系列前腺癌的阳性检出率94.59%,诊断灵敏度97.30%(P<0.05)。结论:前列腺癌分期诊断中CT+3.0T MR LAVA+DWI系列可有效实现疾病分期诊断。

Objective:To evaluate the value of 128-slice spiral CT combined with 3.0T MR LAVA(multi-phase dynamic enhancement)and DWI series in staging of prostate cancer.Methods:A total of 37 patients diagnosed with prostate cancer by prostate specific antigen and rectal digital examination in the clinical laboratory of Yangchun People's Hospital from June 2021 to March 2022 were selected as the study subjects.CT+3.0T MR LAVA(multi-stage dynamic enhancement)and CT+3.0T MR LAVA(multi-stage dynamic enhancement)+DWI series examinations were used,respectively,to observe the diagnostic effects of the two examination methods on stage A,B,C,and D patients.Results:The staging results of CT+3.0T MR LAVA for A,B,C,and D were lower than those of CT+3.0T MR LAVA+DWI series(P>0.05);Among 37 patients with prostate cancer,28 were positive and 9 were negative for CT+3.0T MR LAVA,and 36 were positive and 1 was negative for CT+3.0T MR LAVA+DWI series.The positive detection rate of CT+3.0T MR LAVA prostate cancer was 51.35%,with a sensitivity of 75.66%,which was lower than the positive detection rate of CT+3.0T MR LAVA+DWI series prostate cancer,which was 94.59%,with a diagnostic sensitivity of 97.30%(P<0.05).Conclusion:CT+3.0T MR LAVA+DWI series can effectively achieve disease staging diagnosis in prostate cancer staging diagnosis.
