经鼻高流量湿化治疗仪联合支气管镜肺泡灌洗在重症肺炎合并低氧血症患者中的临床效果分析

摘要 目的:探究重症肺炎合并低氧血症采取经鼻高流量湿化治疗仪结合支气管镜肺泡灌洗(BAL)诊疗的效果。方法:选择2021年1月~2023年1月收治的重症肺炎低氧血症患者80例,采取随机数字法分组,对照组采用单纯BAL治疗,观察组采用BAL+经鼻高流量湿化治疗仪诊疗,比较两组治疗效果、血气指标、炎性因子等。结果:(1)观察组较对照组,治疗有效率获得更高的水平(P<0.05);(2)观察组较对照组,二氧化碳分压(PaCO_(2))更低,氧分压(PaO_(2))、血氧饱和度(SaO_(2))更高(P<0.05);(3)观察组较对照组,C反应蛋白(CRP)、白介素-6(IL-6)等获得更低的水平(P<0.05);(4)观察组较对照组,急性生理学及慢性健康状况Ⅱ获得更低的评分(P<0.05);(5)观察组较对照组重症监护室入住率、气管插管率更低(P<0.05)。结论:重症肺炎合并低氧血症采取经鼻高流量湿化治疗仪结合支气管镜肺泡灌洗诊疗效果确切。 Objective:To explore the effect of nasal high flow humidification therapy combined with bronchoscopy bronchoalveolar lavage(BAL)in the diagnosis and treatment of severe pneumonia with hypoxemia.Methods:80 patients with severe pneumonia and hypoxemia admitted from January 2021 to January 2023 were randomly divided into two groups.The control group received simple BAL treatment,while the observation group received BAL+nasal high flow humidification treatment.The treatment effects,blood gas indicators,inflammatory factors,etc.were compared between the two groups.Results:①The observation group achieved a higher level of treatment effectiveness compared to the control group(P<0.05);②The observation group had lower partial pressure of carbon dioxide(PaCO_(2)),higher partial pressure of oxygen(PaO_(2)),and higher blood oxygen saturation(SaO_(2))compared to the control group(P<0.05);③The observation group showed lower levels of C-reactive protein(CRP)and interleukin-6(IL-6)compared to the control group(P<0.05);④The observation group received lower scores in acute physiology and chronic health status II compared to the control group(P<0.05);⑤The ICU occupancy rate and tracheal intubation rate in the observation group were lower than those in the control group(P<0.05).Conclusion:The diagnosis and treatment of severe pneumonia complicated with hypoxemia by nasal high-flow humidification therapy instrument combined with bronchoalveolar lavage is effective.
