分析内镜下金属钳夹术治疗老年非静脉曲张性上消化道出血的临床价值

To Analyze the Clinical Value of Metal Clamp Under Endoscope for Senile Non-Varicose Upper Gastrointestinal Bleeding

目的:探讨内镜下金属钳夹术治疗老年非静脉曲张性上消化道出血的临床价值。方法:纳入2021年9月~2022年9月本院收治的老年非静脉曲张性上消化道出血患者60例为观察对象,以电脑随机法分为对照组(n=30)、观察组(n=30)两组,分别给予内镜下药物注射治疗及内镜下金属钳夹术治疗,并对比治疗效果。结果:治疗有效率及即时止血率观察组均高于对照组(96.67%,80.00%;90.00%,66.67%),二次出血率观察组低于对照组(3.33%,20.00%),对比均有统计学意义(χ^(2)=4.043、4.811、4.043,P=0.044、0.028、0.044)。出血停止时间、消化功能恢复时间及住院时间观察组均短于对照组[(7.71±1.08),(12.34±2.79)h;(3.32±0.74),(4.21±0.87)d;(4.25±0.33),(5.02±0.46)d],对比均有统计学意义(t=8.477、4.268、7.449,P均=0.000)。结论:内镜下金属钳夹术治疗老年非静脉曲张性上消化道出血的疗效显著,可以加快止血速度,缩短消化功能恢复时间及住院时间。

Objective:To investigate the clinical value of endoscopic metal clamp in the treatment of non-varicose upper gastrointestinal bleeding in the elderly.Methods:The study started and ended from September 2021 to September 2022.60 elderly patients with non-varicose upper gastrointestinal bleeding were randomly divided into control group(n=30),observation group(n=30),after admission.They were given endoscopic drug injection therapy and endoscopic metal clamp therapy respectively,and the therapeutic effects were compared.Results:The treatment effective rate and the rate of immediate hemostasis in observation group were higher than those in control group(96.67%,80.00%;90.00%,66.67%),and the rate of secondary bleeding in observation group was lower than that in control group(3.33%,20.00%).The comparisons were statistically significant(χ^(2)=4.043,4.811,4.043,P=0.044,0.028,0.044).Bleeding cessation time,digestive function recovery time and hospital stay time of observation group were shorter than those of control group[(7.71±1.08),(12.34±2.79)h;(3.32±0.74),(4.21±0.87)d;(4.25±0.33),(5.02±0.46)d].The comparisons were statistically significant(t=8.477,4.268,7.449,P=0.000).Conclusion:Endoscopic metal clamps in the treatment of non-varicose upper gastrointestinal bleeding in the elderly has significant efficacy,which can not only accelerate the hemostasis,but also shorten the time of functional recovery and hospital stay.
