摘要
目的:评价广州市某区学龄儿童第一恒磨牙接受窝沟封闭1年后的防龋效果,比较两种树脂型窝沟封闭剂在临床的使用情况、保留率及其影响因素。方法:随机选取参与广州市免费窝沟封闭项目儿童的第一恒磨牙情况并详细记录,筛选出纳入符合窝沟封闭适应症条件者进行窝沟封闭,1年后复查其保留及龋坏情况。采用SPSS22.0软件对结果进行统计学分析。结果:随机选取500名儿童,经过检查符合该窝沟封闭条件者436名,共计1744颗牙,窝沟封闭1年后复查完整率为82.1%,保留率为94.9%。两种窝沟封闭剂的完整率和保留率虽略有不同,但差异无统计学意义(P>0.05)。各组1年后均无新发龋坏。结论:Kerr和西尔欧(Ascent)两种窝沟封闭剂均能很好起到保护恒牙的作用,复查时均无新发龋坏,1年内完整率和保留率均无明显差异,上颌第一恒磨牙保留率较下颌磨牙稍高。窝沟封闭技术是一种近似于无创操作,通过保护牙釉质不受细菌及其代谢产物的侵蚀并隔绝原有存在于窝沟中细菌的营养来源这两种作用机制,可以效预防乳、恒牙窝沟龋的发生,此方法简单易行值得推广,而封闭剂在其中起到了举足轻重的作用。
Objective:To analyze the caries prevention effect of the first permanent molar of school-age children in a district of Guangzhou after receiving pit and fissure sealing for 1 year,and to compare the clinical use,retention rate and influencing factors of two resin pit and fissure sealing agents.Methods:The first permanent molars of children who participated in the free pit and fissure closure project in Guangzhou were randomly selected and recorded in detail.Those who met the indications for pit and fissure closure were selected for pit and fissure closure,and their retention and caries were rechecked 1 year later.The results were statistically analyzed by SPSS 22.0 software chi square test.There was no new caries in each group after 1 year.Results:500 children were randomly selected.After examination,436 children met the conditions of pit and fissure closure,with a total of 1744 teeth.After 1 year of pit and fissure closure,the integrity rate was 82.1%and the retention rate was 94.9%.Although the integrity rate and retention of the two kinds of pit and fissure sealers were slightly different,the difference was not statistically significant(P>0.05).No new caries occurred in each group after 1 year.Conclusion:Kerr and Ascent,the two kinds of pit and fissure sealers can protect permanent teeth well.There is no new caries during reexamination,and there is no significant difference in integrity rate and retention rate within 1 year.The retention rate of maxillary first permanent molars is slightly higher than that of mandibular molars.Furrow sealing technology is similar to non-invasive operation.It can effectively prevent the occurrence of pit and fissure caries of milk and permanent teeth by protecting the enamel from the erosion of bacteria and their metabolites and isolating the nutritional source of bacteria existing in the pit and fissure.This method is simple and easy,and it is worth popularizing,in which the sealant plays an important role.
