腹腔镜下单孔操作对腹股沟斜疝的治疗效果

The Therapeutic Effect of Laparoscopic Single hole Procedure on Indirect Inguinal Hernia

目的:研讨腹腔镜下单孔操作对腹股沟斜疝患者的治疗效果。方法:选取2020年10月~2022年11月入住本院外科行腹腔镜下全腹膜外疝修补术(TEP)治疗的腹股沟斜疝患者86例,以随机数表法分为两组,每组43例。对照组采用腹腔镜下多孔TEP治疗,观察组行腹腔镜下单孔(SIL-)TEP操作,比较两组手术基本指标(手术时间、术中出血量、住院时间及疼痛评分)、并发症发生情况、复发率及切口满意度。结果:观察组手术时间长于对照组,差异有统计学意义(P<0.05);其余手术基本指标比较,差异无统计学意义(P>0.05)。两组发生切口感染、脐血肿等并发症发生率以及复发率比较,差异无统计学意义(P>0.05)。观察组切口总满意度(93.02%)高于对照组(69.77%),差异有统计学意义(P<0.05)。结论:腹腔镜下SIL-TEP操作用于腹股沟斜疝患者的处理效果确切,虽会延长手术时间,但切口较多孔操作隐蔽,患者满意度较高。

Objective:To investigate the therapeutic effect of laparoscopic single hole procedure on patients with indirect inguinal hernia.Methods:86 patients with indirect inguinal hernia who underwent laparoscopic total extraperitoneal hernia repair(TEP)in our hospital from October 2020 to November 2022 were selected and randomly divided into two groups,with 43 patients in each group.The control group received laparoscopic porous TEP treatment,while the observation group received laparoscopic single hole(SIL-)TEP surgery.The basic surgical indicators(surgical time,intraoperative bleeding,hospital stay,and pain score),complications,recurrence rate,and incision satisfaction were compared between the two groups.Results:The surgical time in the observation group was longer than that in the control group,and the difference was statistically significant(P<0.05);There was no statistically significant difference in other basic surgical indicators(P>0.05).There was no statistically significant difference in the total proportion and recurrence rate of patients with complications such as incision infection and umbilical cord hematoma between the two groups(P>0.05).The total satisfaction with incisions in the observation group(93.02%)was higher than that in the control group(69.77%),with a statistically significant difference(P<0.05).Conclusion:The laparoscopic SIL-TEP procedure is effective in treating patients with indirect inguinal hernia.Although it may prolong the surgical time,the incision is more porous and the patient satisfaction is higher.
