ADC值及IVIM-DWI定量参数对FIGOⅡ期宫颈癌宫旁浸润的诊断效能

Efficacy of ADC Values and IVIM-DWI Quantitative Parameters in the Diagnosis of FIGO II Cervical Cancer Parastatal Invasion

目的:探究磁共振扩散加权成像(DWI)体素内不相干运动(IVIM)模型及表观扩散系数(ADC)单指数模型的定量参数对于FIGOⅡ期宫颈癌患者发生宫旁浸润的诊断效能。方法:回顾性选取2021年1月~2022年12月本院收治的经手术病理学证实为FIGOⅡ期的67例宫颈癌患者,分为未浸润组(n=37)和浸润组(n=30),术前均行常规MRI与多b值DWI检查。分析并评估DWI定量参数对宫颈癌患者发生宫旁浸润的诊断价值。结果:与未浸润组相比,浸润组ADC值、D值均明显较低(P<0.05),f值、D^(*)值则明显较高(P<0.05);其中ADC、D、f、D^(*)及四值联合诊断的ROC曲线下面积分别为0.821、0.890、0.744、0.699和0.974,诊断敏感度、特异度分别为89.2%、63.3%,75.7%、90.0%,76.7%、70.3%,63.3%、73.0%和100%、83.8%。结论:DWI定量参数对FIGOⅡ期宫颈癌发生宫旁浸润具有一定的诊断价值,其中以ADC、D值及四值联合诊断价值较高。

Objective:To explore the diagnostic efficacy of quantitative parameters of magnetic resonance diffusion-weighted imaging(DWI)IVIM model and ADC single index model for FIGO stageⅡcervical cancer patients with parastatal infiltration.Methods:67 patients with cervical cancer confirmed as FIGO stageⅡby surgical pathology admitted to our hospital from January 2021 to December 2022 were retrospectively selected and divided into uninfiltrated group(n=37)and infiltrated group(n=30).Routine MRI and multiple b-value DWI examinations were performed before surgery.To analyze and evaluate the diagnostic value of quantitative parameters of DWI in patients with cervical cancer.Results:Compared with the non-infiltrated group,ADC value and D value in the infiltrated group were significantly lower(P<0.05),while f value and D*value were significantly higher(P<0.05).The area under ROC curve of ADC,D,f,D^(*) and four-value combined diagnosis were 0.821,0.890,0.744,0.699 and 0.974,respectively,and the diagnostic sensitivity and specificity were 89.2%,63.3%,75.7%,90.0%,76.7%,70.3%,63.3%,73.0%,and 100%,83.8%,respectively.Conclusion:Quantitative parameters of DWI have certain diagnostic value for FIGO stageⅡcervical cancer with parastatal invasion,among which ADC,D value and four value combined diagnosis value is higher.
