在鉴别诊断缺血性心肌病与扩张型心肌病中选用彩色多普勒超声检查的临床意义

The Clinical Significance of Color Doppler Ultrasonography in the Differential Diagnosis of Ischemic Cardiomyopathy and Dilated Cardiomyopathy

目的:分析在缺血性心肌病和扩张型心肌病鉴别诊断中采用彩色多普勒超声检查的临床意义。方法:本次研究对象均为2019年8月~2020年4月在本院经临床确诊的缺血性心肌病和扩张型心肌病患者,各44例。所有患者均接受彩色多普勒超声检查,对比分析两组患者的检查结果。结果:扩张型心肌病患者中,35例心室形态表现为球形,缺血性心肌病患者中,31例表现为圆拱门形;在室壁运动上,扩张型心肌病患者31例呈弥漫性减弱,缺血性心肌病患者35例呈节段性减弱,两组有统计学差异(P<0.05)。在心功能上,扩张型心肌病主要为左室收缩功能下降,缺血性心肌病患者主要为舒张功能下降,两组有统计学差异(P<0.05)。在血流动力学上,扩张型心肌病主要为多瓣膜反流,缺血性心肌病患者主要为二尖瓣反流,两组有统计学差异(P<0.05)。结论:缺血性心肌病和扩张型心肌病在彩色多普勒超声检查中,其各项指标存在明显差异,可作为鉴别诊断的有效手段。

Objective:To analyze the clinical significance of color Doppler ultrasonography in the differential diagnosis of ischemic cardiomyopathy and dilated cardiomyopathy.Methods:The subjects of this study were all patients with ischemic cardiomyopathy and dilated cardiomyopathy clinically diagnosed in our hospital,44 cases in each,and the selected time range was from August 2019 to April 2020.All patients underwent color Doppler ultrasonography,and the results of the two groups of patients were compared and analyzed.Results:Among the patients with dilated cardiomyopathy,35 patients with dilated cardiomyopathy showed spherical shape,and 31 patients with ischemic cardiomyopathy showed round arch shape.35 patients with ischemic cardiomyopathy showed segmental weakening,and there was statistical difference between the two groups(P<0.05).In terms of cardiac function,dilated cardiomyopathy mainly had a decrease in left ventricular systolic function,while patients with ischemic cardiomyopathy mainly had a decrease in diastolic function,and there was a statistical difference between the two groups(P<0.05).In terms of hemodynamics,dilated cardiomyopathy was mainly multivalve regurgitation,while ischemic cardiomyopathy patients were mainly mitral regurgitation,and there was a statistical difference between the two groups(P<0.05).Conclusion:Color Doppler ultrasonography has significant differences in the indicators of ischemic cardiomyopathy and dilated cardiomyopathy,which can be used as an effective means of differential diagnosis.
