基于中医传承辅助平台分析戴淑青治疗耳鸣用药规律

An analysis of Professor DAI Shuqing’s medication rule in the treatment of tinnitus based on the Traditional Chinese Medicine Inheritance Support System

目的:总结名老中医戴淑青教授治疗耳鸣的用药规律,为临床用药和研究提供参考。方法:收集2021年3月-2022年4月戴淑青教授于青岛市中医医院治未病科门诊治疗耳鸣的患者病例,筛选后纳入有效患者100例,首诊处方100首。参照《中药学》,对原始数据进行核对,将名称进行统一规范。为保证数据准确严谨,由另外的研究者对方剂进行校对。运用中医传承辅助平台(V2.5)对高频药物、四气五味、归经、药物组合、关联规则、新方分析等进行数据挖掘,总结戴淑青教授治疗耳鸣的用药规律。结果:共筛选出处方100首,包含中药106味。其中川芎、柴胡、炙甘草、葛根、茯神、石菖蒲、磁石等使用频次最高;所用中药四气以温、寒、平为主;五味以辛与甘为主;主要归心、肝、脾、肾四经。经关联规则分析,提取到25个常用药物组合,频次较高的有川芎-柴胡、炙甘草-川芎等;由熵聚类分析得到6个潜在新方。结论:戴淑青教授治疗耳鸣从心、肝、脾、肾四脏入手,注重养心神,调气机,善用行气血升清阳之药,以和解少阳、疏肝健脾、补肾通窍为基本治法。

Objective:To summarize the medication rule of Professor DAI Shuqing in treating tinnitus,and to provide reference for clinical medication and research.Methods:Firstly,we collected the cases of patients with tinnitus treated by Professor DAI Shuqing in the outpatient department of Qingdao Traditional Chinese Medicine Hospital from March 2021 to April 2022.After screening,we included 100 effective patients and 100 first diagnosis prescriptions.With reference to Chinese Materia Medica,we checked the original data and standardize the name.In order to ensure the accuracy and preciseness of the data,another researchers proofread the prescription.The Traditional Chinese Medicine Inheritance Support System(TCMISS)V2.5 was used to conduct data mining on highfrequency drugs,four Qi(气)and five flavors,meridians,drug combinations,association rules,new prescription analysis,etc..Results:A total of 100 prescriptions were screened out,including 106 TCM medicines.Among them,Chuanxiong(Rhizoma Chuanxiong),Chaihu(Radix Bupleuri),Zhigancao(Radix Glycyrrhizae),Gegen(Radix Puerariae Lobatae),Fushen(Poria cum Radix Pini),Shichangpu(Rhizoma Acori Tatarinowii)and Cishi(Magnetitum)were the most frequently used.The five flavors are mainly pungent and sweet,and mainly belong to the four meridians of the heart,liver,spleen and kidney.After association rule analysis,25 commonly used drug combinations were extracted,and 6 potential new prescriptions were obtained by entropy cluster analysis.Conclusion:Professor DAI Shuqing’s treatment of tinnitus starts from the four internal organs of the heart,liver,spleen and kidney,pays attention to nourishing the mind,regulates Qi,and makes good use of the medicines that move Qi and blood to promote Yang(阳).
