中医药治疗月经过少用药规律的数据挖掘研究

A study on the medication rule of TCM in treating hypomenorrhea based on data mining

目的:采用数据挖掘方法,总结中药汤剂治疗月经过少的用药规律。方法:检索中国知网数据库,选取2000年-2022年以中药汤剂为治疗方法治疗月经过少的临床研究类文献,并将资料录入中医传承计算平台(V3.0),利用软件的数据挖掘方法,对最终纳入的中药进行频次、归经、性味、类别的统计,对高频药物进行关联规则分析和聚类分析。结果:本研究共筛选处方共计159首,涉及中药183味,高频药物24味(频次≥30),排名居前5位的中药为当归、熟地黄、白芍、菟丝子、川芎;五味以甘味药最多,四气药次之;五味以温为主;归经以入肝经、脾经、肾经为主;药物以补虚药为主,活血化瘀药、清热药、理气药次之;高频药物组合居前5位的分别为当归-白芍、当归-熟地黄、当归-川芎、当归-菟丝子、熟地黄-菟丝子;药物关联规则有33条,核心药物组合包括白芍,川芎→当归、白芍,香附→当归、川芎,甘草→当归、川芎→当归;共有3个聚类方,聚类1:当归、菟丝子、熟地黄、白芍、枸杞子;聚类2:当归、白芍、川芎、甘草、熟地黄;聚类3:当归、川芎、甘草、白芍、赤芍。结论:现代医家对于月经过少的治疗多以四物汤为核心基础方,在辨证论治的基础上配合以补血化瘀、补肝益肾、疏肝理气为主的药物。这一结果或可为临床治疗月经过少提供一定的思路。

Objective:To summarize the medication rule of TCM decoction in the treatment of hypomenorrhea by data mining method.Methods:In CNKI database,clinical research literature on treating hypomenorrhea with TCM decoction from 2000 to 2022 was selected.The relevant data was entered into the traditional Chinese medicine inheritance calculation platform(V3.0),and the data mining method of the software was used to count the medication frequency,meridian tropism,medicinal property and taste,and category of the included TCM medicines.The association rule analysis and cluster analysis of high-frequency medicines were carried out finally.Results:A total of 159 prescriptions were screened in this study,involving 183 TCM medicines and 24 high-frequency medicines(the medication frequency was at least 30 times),and the top 5 TCM medicines were Danggui(Radix Angelicae Sinensis),Shu Dihuang(Radix Rehmanniae Preparata),Baishao(Radix Paeoniae Alba),Tusizi(Semen Cuscutae),Chuanxiong(Rhizoma Chuanxiong).The medicine mostly used was sweet medicine,followed by bitter medicine.The medicinal property was mainly warm.The meridian tropism was mainly liver,spleen and kidney meridians.The medicines mostly used were tonify medicines,followed by blood circulating and stasis removing medicine,heat clearing medicine,and Qi(气)regulating medicine.The top 5 high-frequency medicine combinations were Danggui-Baishao,Danggui-Shu Dihuang,Danggui-Chuanxiong,Danggui-Tusizi,Shu Dihuang-Tusizi.There were 33 medicine associated rules,and the core medicine combinations include Baishao plus Chuanxiong→Danggui,Baishao plus Xiangfu(Rhizoma Cyperi)→Danggui,Chuanxiong plus Gancao(Radix Glycyrrhizae)→Danggui,Chuanxiong→Danggui.There were 3 cluster prescriptions,cluster 1:Danggui,Tusizi,Shu Dihuang,Baishao,Gouqizi(Fructus Lycii),cluster 2:Danggui,Baishao,Chuanxiong,Gancao,Shu Dihuang,cluster 3:Danggui,Chuanxiong,Gancao,Baishao,Chishao(Radix Paeoniae Rubra).Conclusion:Modern physicians mostly select the Siwu decoction(四物汤)as the core basic method for the treatment of hypomenorrhea,which is combined with the treatment based on syndrome diffenentiation to replenish blood and remove blood stasis,replenish liver and kidney,and soothe liver and regulate Qi.This result may provide some ideas for the clinical treatment of hypomenorrhea.
