火龙罐疗法治疗社区获得性肺炎(痰湿阻肺证)患者临床观察

Clinical Observation of Huolong Cupping in Patients with CAP in Syndrome of Phlegm-dampness Obstructing Lung

目的观察火龙罐疗法治疗社区获得性肺炎(CAP)(痰湿肺阻证)患者的临床效果并探讨其作用机制。方法70例患者按随机数字表法分为观察组与对照组各35例。对照组予抗感染、补液、化痰、止咳等对症支持处理,并予常规临床护理措施。观察组在对照组基础上加用火龙罐治疗。7 d为1个疗程,两组均治疗7 d。比较两组治疗7 d后咳嗽症状积分、莱切斯特咳嗽生活质量问卷(LCQ)评分、中医证候评分、血清炎性介质C反应蛋白(CRP)、白细胞计数(WBC)、中性粒细胞绝对值(NE#)、中性粒百分比(NE%)、降钙素原(PCT)浓度及临床疗效。结果两组治疗后日间咳嗽积分、夜间咳嗽症状积分和咳嗽总积分与本组治疗前比较均下降,且观察组评分均低于对照组(P<0.05)。两组治疗后生理、心理、社会及总LCQ评分与本组治疗前比较均升高,且观察组评分均高于对照组(P<0.05)。观察组总有效率为94.28%,高于对照组的68.57%(P<0.05)。两组治疗后各中医证候评分与治疗前比较,均下降(P<0.05),且观察组各中医证候评分均低于对照组(均P<0.05)。两组治疗后各炎性反应递质与治疗前比较,均下降(P<0.05),且观察组炎性反应递质指标水平均低于对照组(均P<0.05)。两组患者胸部CT改善情况差别不大(P>0.05)。两组均未出现任何皮肤瘙痒、红疹、水泡、烫伤等不良反应。结论火龙罐治疗CAP可明显减轻患者的咳嗽症状,降低炎性指标,提高治疗总有效率、中医证候评分及生活质量,改善预后,安全可靠。

Objective:To observe the clinical effect of Huolong Cupping therapy in the treatment of patients with community-acquired pneumonia(CAP)with syndrome of phlegm-damp obstructing lung.Methods:A total of 70 patients were divided into observation group and control group according to the random number table meth⁃od,with 35 cases in each group.Both groups were treated with symptomatic support such as anti-infection,fluid re⁃placement,phlegm dissolution,and cough suppression,and routine clinical nursing measures,and the observation group was treated with Huolong Cupping on the basis of the control group.The treatment course of both groups was 7 days as 1 course of treatment.The scores of cough symptoms,Leicester cough quality of life questionnaire(LCQ)score,TCM symptom score,serum inflammatory mediator[C-reactive protein(CRP),white blood cells(WBC),absolute neutrophil(NE#),neutrophil percentage(NE%),procalcitonin(PCT)]and clinical efficacy were compared after 7 days of treatment.Results:After 7 days of treatment,the cough symptom score,TCM symptom score,C-reactive protein(CRP),white blood cells(WBC),absolute neutrophil value(NE#),neutrophil percentage(NE%)and procalcitonin(PCT)decreased compared with the pre-treatment data(P<0.05),and the cough symp⁃tansscores of the observation group after treatment were lower than those in the control group(P<0.05).The LCQ total score was higher than that before treatment(P<0.05),and the LCQ scores of the experimental group after the intervention were higher than those in the control group(P<0.05).The total clinical effective rate of the observa⁃tion group(94.28%)was higher than that of the control group(68.57%)(P<0.05).There was no statistical differ⁃ence in the improvement of chest CT in the two groups(P>0.05).There were no adverse reactions.Conclusion:Huolong Cupping treatment of CAP can significantly reduce the clinical symptoms of cough,reduce inflammatory indexes,improve the overall effective rate of treatment,the score of TCM syndrome and quality of life,improve prognosis.It is safe,reliable and is suitable for clinical application and promotion.
