探讨阴道镜下宫颈活检在宫颈癌中的诊断价值

Diagnostic Value Exploration of Cervical Biopsy Under Colposcopy for Cervical Cancer

目的探讨阴道镜下宫颈活检在宫颈癌中的诊断价值。方法将2022年4月—2023年1月在本检验公司检验的98例疑似宫颈癌患者的病理标本作为研究对象,病理标本的获取途径为阴道镜下宫颈活检标本及宫颈环形电切术后病理标本,前者作为观察组,后者作为对照组,两者联合作为联合组,以临床诊断结果为金标准,对比三组不同宫颈病变的诊断符合率,分析与临床诊断结果的对照,计算诊断效能。结果联合组对高级别上皮内瘤变、浸润癌的诊断符合率明显高于观察组、对照组(P<0.05),三组对慢性宫颈炎、尖锐湿疣/伴有HPV感染、低级别上皮内瘤变的诊断符合率相当(P>0.05);联合组中真阳性、真阴性例数高于观察组、对照组,与临床诊断结果的符合度最高(P<0.05);联合组对宫颈癌诊断的敏感度、特异度、准确度、阳性预测值、阴性预测值均高于观察组、对照组(P<0.05)。结论阴道镜下宫颈活检在宫颈癌中的诊断价值一般,联合宫颈环形电切术后病理检查能提高对宫颈癌的诊断效果。

Objective To investigate diagnostic value of cervical biopsy under colposcopy for cervical cancer.Methods The paper chose pathological specimens of 98 patients with suspected cervical cancer in our laboratory from April 2022 to January 2023 as research subjects,access of pathological specimens was cervical biopsy specimens under colposcopy and cervical circular electrosection,the former was included in observation group,and the latter in control group,combination of two groups in combined group.With gold standard of clinical diagnosis results,diagnostic accuracy of different cervical lesions between two groups was compared,comparison with clinical diagnosis results was analyzed to calculate diagnostic effect.Results Diagnostic coincidence rate of high-grade intraepithelial neoplasia and invasive cancer in combined group was significantly higher than observation group and control group(P<0.05).Diagnostic coincidence rate of chronic cervicitis,genital wart/with hpv infection and low-grade intraepithelial neoplasia among three groups was similar(P>0.05).Number of true positive and true negative cases in combined group was higher than observation group and control group,consistency with clinical diagnosis results was the highest(P<0.05).Sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined group in diagnosis of cervical cancer was higher than observation group and control group(P<0.05).Conclusion Cervical biopsy under colposcopy is of general value in diagnosis of cervical cancer,combined with pathological examination after cervical circular electrosection can improve diagnostic effect of cervical cancer.
