血管超声对下肢骨折患者下肢静脉血栓的诊断价值探讨

Diagnostic Value Discussion of Vascular Ultrasound for Lower Limb Venous Thrombosis of Patients with Lower Limb Fractures

目的 研究分析血管超声对下肢骨折患者下肢静脉血栓的诊断价值。方法 选择2021年9月—2022年9月本院治疗的下肢骨折患者82例为研究对象,所有患者均给予血管超声检查,将未检出下肢静脉血栓的41例患者设为对照组,将检出下肢静脉血栓的41例患者设为观察组,对两组患者的超声血流动力学指标的检查结果进行统计比较,同时总结分析下肢静脉血栓发生情况、形成因素及形成部位。结果 观察组收缩期峰值流速(Vs)、舒张末期流速(Vd)高于对照组,血流阻力指数(RI)低于对照组,对比差异存在统计学意义(P<0.05)。在观察组中,急性期血栓20例,亚急性期血栓11例,慢性期血栓10例;血栓形成因素中,骨折及骨科相关疾病19例,术后长期卧床9例,浅静脉曲张5例,糖尿病、肥胖、高血压4例,其他原因4例;血栓形成部位中,左侧29例,右侧11例,双侧1例,股静脉合并腘静脉血栓32例,股静脉血栓5例,腘静脉血栓4例。结论 在下肢骨折患者下肢静脉血栓的诊断中,血管超声检查的应用价值非常高,具有操作简单、定位准确、安全、无创等优势,能够准确判断血栓发生情况、形成因素及形成部位,为临床诊断与治疗提供了可靠的指导依据。

Objective To study and analyze diagnostic value of vascular ultrasound for lower limb venous thrombosis of patients with lower limb fractures.Methods The paper chose 82 patients with lower limb fractures in our hospital from September 2021 to September 2022 as research subjects,with vascular ultrasound examination.41 cases in control group showed no lower limb venous thrombosis,and 41 cases in observation group had lower limb venous thrombosis.Ultrasound hemodynamic indicators between two groups were statistically compared,occurrence of lower limb venous thrombosis was summarized and analyzed formation factors and location.Results Peak systolic flow velocity(Vs)and end diastolic flow velocity(Vd)of observation group was higher than control group,while blood flow resistance index(RI)was lower than control group(P<0.05),indicating statistical differences.in observation group,there were 20 cases of acute phase thrombosis,11 cases of subacute phase thrombosis,and 10 cases of chronic phase thrombosis.Among thrombosis factors,there were 19 cases of fracture and orthopedic related diseases,9 cases of long-term bed rest after surgery,5 cases of superficial varicose veins,4 cases of diabetes,obesity,hypertension,and 4 cases of other causes.Among the thrombosis sites,there were 29 on the left,11 on the right,and 1 on both sides.There were 32 cases of femoral vein thrombosis combined with popliteal vein thrombosis,5 cases of femoral vein thrombosis,and 4 cases of popliteal vein thrombosis.Conclusion Vascular ultrasound examination has high application value in diagnosis of lower limb vein thrombosis of patients with lower limb fractures,with advantages of simple operation,accurate positioning,safety,and non-invasive,can determine occurrence,formation factors and location of thrombosis accurately,which provides reliable guidance for clinical diagnosis and treatment.
