低分子肝素钠联合孕激素治疗妊娠早期先兆流产的临床效果分析

Clinical Analysis of Low Molecular Weight Heparin Sodium Combined with Progesterone in the Treatment of Threatened Miscarriage in Early Pregnancy

目的观察低分子肝素钠联合孕激素治疗妊娠早期先兆流产的临床效果。方法选取2020年6月至2022年6月在本院接受治疗的80例妊娠早期先兆流产患者,随机分为对照组与观察组,每组40例。对照组患者给予孕激素治疗,观察组患者在使用孕激素药物治疗的基础上联合低分子肝素钠治疗。对比两组超声指标、血清水平、免疫功能、炎性症状、临床证候积分、产后并发症发生率。结果观察组患者治疗后子宫螺旋动脉阻力指数、搏动指数均低于对照组(P<0.05)。观察组患者治疗后血清孕酮、雌二醇、人绒毛膜促性腺激素、孕激素诱导的阻断因子水平均高于对照组(P<0.05)。观察组患者治疗后IgG、IgA、IgM水平均高于对照组(P<0.05)。观察组患者治疗后IL-2、IL-4、INF-γ水平、临床证候积分均低于对照组(P<0.05)。观察组产后并发症发生率(10.00%)低于对照组(32.50%),P<0.05。结论低分子肝素钠联合孕激素治疗妊娠早期先兆流产效果切实,能够促进患者性激素的分泌、增强机体免疫力,减少产后并发症发生率。

Objective To observe the clinical effect of low molecular weight heparin sodium combined with progesterone in the treatment of threatened miscarriage in early pregnancy.Methods Eighty patients with early threatened abortion during pregnancy who received treatment in our hospital from June 2020 to June 2022 were randomly divided into a control group and an observation group,with 40 patients in each group.The control group patients were treated with progesterone alone,while the observation group patients were treated with low-molecular-weight heparin sodium on the basis of progesterone drugs.Ultrasound indicators,serum levels,immune function,inflammatory symptoms,clinical syndrome scores,and postpartum complications were compared between the two groups under different drug treatment modes.Results After treatment,the resistance index and pulsatility index of the uterine spiral artery in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of serum progesterone,estradiol,human chorionic gonadotropin,and progesterone induced blocking factors in the observation group were higher than those in the control group(P<0.05).The levels of IgG,IgA,and IgM in the observation group were higher than those in the control group after treatment(P<0.05).After treatment,the levels of IL-2,IL-4,INF-γ,and clinical syndrome scores of the observation group were lower than those of the control group(P<0.05).The incidence of postpartum complications in the observation group(10.00%)was lower than the control group(32.50%),P<0.05.Conclusions The combination of low-molecular-weight heparin sodium and progesterone is effective in treating threatened miscarriage in early pregnancy.It can promote the secretion of sex hormones,enhance the body's immunity,and reduce the incidence of postpartum complications.
