PFNA术中股骨大转子进针点修正处理的效果分析

Analysis of the Effect of Correcting the Insertion Point of the Femoral Trochanter During PFNA Surgery

目的分析PFNA术中股骨大转子进针点修正处理的临床效果。方法参与本次研究的股骨粗隆骨折患者一共144例,均选自我院骨科2021年6月至2022年12月期间收治的患者。按照随机数字表法将144例患者分为参考组及试验组。72例参考组患者行传统的进针方法治疗,72例试验组患者采取改良股骨大转子进针点处理。比较两组效果。结果手术时间、术中出血量、康复下地时间对比显示,试验组低于参考组(P<0.05)。两组骨折对位对线良好率比较,试验组高于参考组(P<0.05)。两组髋关节功能情况对比发现,治疗前两组比较差异无统计学意义(P>0.05),治疗后试验组高于参考组(P<0.05)。试验组及参考组并发症发生率分别为4.17%、13.89%,对比并发症发生率发现,试验组低于参考组(P<0.05)。对比两组生活质量发现,试验组高于参考组(P<0.05)。结论股骨粗隆骨折PFNA术中采用改良股骨大转子进针点法与传统进针方法相比,骨折对位对线优良率明显提高,缩短患者的手术时间、康复下地时间,减少术中出血量,促进其髋关节功能恢复,减少并发症,从而提升患者的生活质量。

Objective To analyze the clinical effect of correcting the insertion point of the greater trochanter of the femur during PFNA surgery.Methods One hundred and forty-four patients with femoral trochanteric fractures admitted to the department of orthopedics,our hospital from June 2021 to December 2022 were selected as the study subjects.They were randomly divided into two groups,namely the reference group and the experimental group,with the reference group treated with traditional needle insertion methods and the experimental group treated with improved trochanteric needle insertion points.There are 72 cases in each group.Compare the effects of the two groups.Results The comparison of operation time,intraoperative blood loss and recovery time in experimental group was lower than that in ref erence group(P<0.05).The good fracture alignment rate of experimental group was higher than that of reference group(P<0.05).The comparison of hip joint function between the two groups showed a small difference before treatment(P>0.05),after treatment,the experimental group was higher than the reference group(P<0.05).The incidence of complications in the experimental group and the reference group was 4.17%and 13.89%,respectively.The comparison of the incidence of complications showed that the experimental group was lower than the reference group(P<0.05).The comparison of quality of life showed that the experimental group was higher than the reference group(P<0.05).Conclusions Compared with traditional needle insertion methods,the improved femoral trochanter insertion point method used in PFNA surgery for femoral trochanteric fractures significantly improves the excellent rate of fracture alignment and alignment,shortens the patient's surgical time,rehabilitation time,reduces intraoperative bleeding,promotes hip joint function recovery,reduces complications,and improves the patient's quality of life.
