TCT联合高危型HPV检测对社区宫颈癌筛查的效果评价

Evaluation of TCT Combined with High-Risk HPV Detection in Community Cervical Cancer Screening

目的分析宫颈液基细胞学检查(TCT)联合高危型人类乳头瘤病毒(HPV)检查在社区宫颈筛查中的效果。方法选择750例使用TCT联合高危型HPV检测筛查患者作为研究对象,病例收集时间为2021年1月至2022年8月,对筛查结果异常者建议去上级医院进行阴道镜及活检检查,统计分析比较单纯TCT筛查或HPV检测与TCT联合HPV筛查的病理活检情况。结果共筛查750例患者,筛查结果异常者83例,根据宫颈癌筛查诊断“三阶梯”原则建议患者去上级医院进行阴道镜及病理活检,单纯使用TCT筛查阳性率为14.46%,单纯使用HPV筛查阳性率为20.48%,TCT联合HPV筛查阳性率为26.51%。结论TCT联合高危型HPV检测可以提高筛查阳性率,在宫颈病变筛查中有重要作用,比单纯以TCT筛查或HPV检测作为初筛方法能更好地降低宫颈高度病变的漏诊率,尽早发现病变。

Objective To analyze the effect of cervical fluid based cytology(TCT)combined with high-risk human papillomavirus(HPV)in community cervical screening.Methods Seven hundred and fifty patients who were screened by TCT combined with high-risk HPV detection were selected as the study objects,and the collection time was from January 2021 to August 2022.Patients with abnormal screening results were recommended to go to superior hospitals for colposcopy and biopsy.The pathological biopsies of TCT screening alone or HPV screening combined with TCT were compared statistically.Results A total of 750 patients were screened,83 of which had abnormal screening results.According to the"three-step"principle of cervical cancer screening and diagnosis,patients were recommended to go to superior hospitals for colposcopy and pathological biopsy.The positive rate of TCT screening alone was 14.46%,and that of HPV screening alone was 20.48%.The positive rate of TCT combined with HPV screening was 26.51%.Conclusions TCT combined with high risk HPV detection can improve the positive rate of screening and play an important role in screening cervical lesions.Compared with TCT screening or HPV detection as the primary screening means,it can better reduce the missed diagnosis rate of high cervical lesions and detect lesions as early as possible.
