探讨斜视手术中发生眼心反射的影响因素分析

Exploring the Analysis of Factors Influencing the Occurrence of Oculocen⁃tric Reflex in Strabismus Surgery

目的 探究斜视手术中影响眼心反射(oculocardiac reflex, OCR)因素。方法 回顾性分析2021年2月—2022年10月厦门大学附属厦门眼科中心收治的500例斜视手术患者临床资料,患者均常规行斜视矫正术,统计术中发生OCR的情况,采用单因素分析、多因素Logistic回归分析探究影响斜视手术患者发生OCR影响因素。结果 单因素分析显示,不同年龄、麻醉方式、阿托品预处理、联合球后麻醉的斜视手术患者的OCR发生率比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,年龄是斜视手术患者发生OCR的独立危险因素(OR=2.524,P<0.05),阿托品预处理、联合球后麻醉是其保护因素(OR=0.425、0.370,P<0.05)。结论 年龄、阿托品预处理、联合球后麻醉可影响斜视术中OCR发生,麻醉期间予以阿托品预处理或联合球后麻醉对降低OCR发生有积极意义,同时对不同年龄段患者应加以重视。

Objective To investigate the factors affecting oculocardiac reflex(OCR)in strabismus surgery.Methods Retrospectively analyzed the clinical data of 500 strabismus surgery patients admitted to Xiamen Eye Center affiliated to Xiamen University from February 2021 to October 2022,who were routinely operated with strabismus correction,and counted the occurrence of OCR during the operation.Univariate analysis and multifactorial logistic regression analysis were used to explore the factors affecting the occurrence of OCR in patients with strabismus surgery.Results Univariate analysis showed that there were statistically significant differences in the incidence of OCR among strabismus patients with different ages,anesthesia methods,atropine pretreatment and combined with retrobulbar anesthesia(P<0.05).Multifactorial logistic regression analysis showed that age was an independent risk factor for OCR in patients undergoing strabismus surgery(OR=2.524,P<0.05),and atropine pretreatment and combined post-balloon anesthesia were its protective factors(OR=0.425,0.370,P<0.05).Conclusion Age,atropine pretreatment,and combined post-bulbar anesthesia can affect the occurrence of OCR during strabismus surgery,and atropine pretreatment or combined post-bulbar anesthesia during anesthesia has a positive significance in reducing the occurrence of OCR,and at the same time,it should be paid attention to the patients of different age groups.
